Spoilers for Smile (2022) and Smile 2. Rose does not escape the entity. Her apparent victory at the cabin is a hallucinated reprieve; when Joel arrives, he witnesses her death, and the ending signals that the curse has passed to him. Smile 2 continues the story of the curse with a new protagonist, pop star Skye Riley.
What happens to Rose at the end of Smile?
Rose goes to the isolated cabin hoping that, if she dies without anyone witnessing it, the chain of deaths might stop. She appears to confront the entity, set it on fire, and leave for Joel’s apartment. That apparent escape is a false ending: Rose has not left the cabin, and the entity still has control of her.
Joel arrives and sees Rose set herself alight. The film’s final reveal makes clear that Rose dies at the cabin, rather than defeating the entity or surviving to reunite with Joel. TIME’s October 1, 2022 ending explainer describes the sequence and its reversal.
Does the curse pass to Joel?
The ending signals that Joel is the next person exposed to the curse because he witnesses Rose’s death. The film does not show what happens to him afterward or specify how much time he has. It leaves his fate beyond the first film’s final scene.

Why Rose’s apparent victory matters
Rose’s false victory makes the ending tragic rather than triumphant. The cabin confrontation brings her childhood trauma into view: she remembers her mother’s suicide and her own memory of not responding to her mother’s plea. Rose appears to confess and reach catharsis, but the emotional release does not erase the pain or prevent the entity’s return.
Writer-director Parker Finn described the way unresolved pain can return: “I think those kinds of things are pernicious. They linger, they don’t let go. They creep back, whether we want them or not.” The remark appears in TIME’s October 1, 2022 explainer and interview.

Is the entity literal, or a symbol of trauma?
The film supports both a literal supernatural reading and a psychological one. On the plot level, the entity is real and the ending suggests it moves through people who witness a death. Thematically, it can also be read as an external form of Rose’s trauma and the return of unresolved pain.
Finn has said he wanted to place a grounded character with internal struggles inside a cursed-chain story, while leaving room for uncertainty about what is real. Those readings need not cancel each other out: the supernatural events can drive the plot while also expressing Rose’s subjective experience. In a November 2022 interview, Finn called the original film self-contained: “Smile was always designed to just exist for its own right, to be very self-contained and to tell the whole story.” He also left elements of the world open for future exploration and said he wanted a follow-up to feel fresh and unexpected. SYFY WIRE’s interview with Finn discusses the film’s ending and sequel possibilities.

How the ending connects to Smile 2
Smile 2 is a released follow-up, not a hypothetical continuation. Paramount’s film page dates it October 18, 2024, credits Finn as writer-director, and introduces pop star Skye Riley, who begins experiencing frightening events before a world tour. The Directors Guild of America also describes it as the follow-up to the 2022 film: DGA announcement.
The first film’s ending supplies a clear bridge: Joel witnesses Rose’s death, and the curse appears to move on. That continuation does not undo Rose’s fate. Her story ends at the cabin; the franchise can continue through the danger now facing another character.
