Ryan Murphy Breaks Down Grotesquerie’s Mind-Bending Twist

Episode Seven reveals that Lois’s murder investigation was a coma experience—but the season-one finale leaves the killer unnamed and reality uncertain.

Spoilers for Grotesquerie through the season-one finale. Episode Seven reveals that Lois Tryon has been unconscious in a hospital: the murder investigation that occupied the first six and a half episodes was a vivid coma experience. Ryan Murphy has said the twist was built into the series from the start. But it is a reveal, not a definitive explanation of every event that follows: the finale leaves Lois questioning whether she has truly woken up, and does not name the killer.

What is the Grotesquerie twist?

For much of the first six and a half episodes, Grotesquerie appears to be a crime drama about Detective Lois Tryon investigating a series of disturbing murders alongside Sister Megan. Episode Seven overturns that premise: Lois is in a hospital, unconscious, and the investigation has unfolded as a vivid coma experience. Sister Megan’s apparent connection to the murders belongs to that experience, rather than serving as the straightforward solution to a real-world case.

The reveal changes how viewers read the show’s earlier scenes. It does not, on its own, prove that everything after Lois’s apparent awakening is unreal. The distinction between the coma world and what follows remains part of the story’s uncertainty.

How Ryan Murphy says the twist was designed

Murphy told TheWrap that the twist was “always” part of the series. He described the production as unusually meticulously designed, saying, “I’ve never done anything that worked out in advance, that meticulously designed.” TheWrap also reported that FX chairman John Landgraf likened the show to a Russian nesting doll, with meanings beyond the first reveal.

FX’s official behind-the-scenes feature describes clues layered into scenes before Episode Seven. TheWrap identifies several examples:

  • The burning curtain: The opening image echoes the curtains surrounding Lois’s hospital bed.
  • Nurse Redd’s cherries: Her choice of cherries rather than strawberries gestures toward Lois’s subconscious fear of blood clots.
  • Sudden white light: The light signals Lois ascending to a “higher plane.”
  • Eddie’s introduction: The first meeting frames him as a guardian angel.

Those details support Murphy’s account of a planned reveal; they need not make every later ambiguity resolve into a single, literal answer.

Murphy’s larger question: Is this really happening?

In Vanity Fair, Murphy said he wanted to write about feelings he and his collaborators shared about the surrounding world, including the questions, “Are we in a simulation? When are people going to wake up?” He summed up the show’s broad question as: “No matter what side you’re on or who you are, the question is, is this really happening?”

That is Murphy’s framing of the series, not a final answer about its reality layers. The coma reveal gives viewers a new way to interpret the investigation, while the show’s broader question keeps the boundary between dream and waking life unsettled.

What the finale does—and does not—resolve

Episode Seven’s coma reveal and the season-one finale’s developments are separate story beats. In the finale account from Radio Times, Lois wakes into a world where echoes of the dream begin to appear. She worries that she may be in another dream layer.

The finale does not identify Grotesquerie. Lois says she will tell her colleagues the killer’s identity later, and the Radio Times account does not disclose a name. So the season offers a major explanation for the earlier episodes without delivering a named killer as its final answer.
