In Never Let Go, Momma cuts her own throat after her rope is cut and an apparition approaches. Director Alexandre Aja said the shocking moment was always in the screenplay and was what the story was building toward. But his comments explain the twist’s role in the film—not whether the evil is objectively real.

What happens to Momma at the end?
After one of her sons cuts her rope, Momma is confronted by apparitions of people she killed while trying to protect her children, including her husband. When one comes closer, she uses broken glass to cut her throat rather than let it touch her. That account of the scene was reported by TheWrap on September 20, 2024.
The moment overturns the family’s established survival rule: stay connected to the house by rope, because danger waits beyond it. Once the rope is severed, the film offers no reassuring explanation for what Momma sees or a way to tell whether her terror is justified.

What did Alexandre Aja say about the twist?
In his interview with TheWrap, Aja said, “The script had a few twists but this one is the one I definitely did not see coming.” He also said the moment was “always in the script,” and that he was “happy that we didn’t have to change that.” TheWrap reports that he described it as what the movie was “building up to.”
His comments point to the scene’s structural purpose: it strips away the audience’s sense of security and destabilizes the story. They do not establish that Aja intended the apparitions as proof of a literal entity, nor do they provide a definitive explanation of the ending.

Is the evil real, or is it in Momma’s mind?
The ending supports two readings, and neither is confirmed as the answer. A secondary interpretation in Plot Explained’s June 25, 2025 account likewise treats the final events as unresolved rather than as an official explanation.
A supernatural reading
The apparitions and the film’s physical clues can be read as signs that a real force threatens the family. On this reading, Momma’s rope rule is a response to an actual danger, and the closing image of Nolan suggests the evil may still be present after her death.

A psychological reading
The same events can be read through the family’s fear and trauma. Momma’s certainty shapes what her sons believe and how they behave; her death removes the person who has been their main source of information about the outside threat. Nolan’s final moments may suggest possession, but suggestion is not confirmation.
The rope therefore functions both as a survival practice within the story and as a sign of how completely Momma’s fear governs the family. That is an interpretation of the film’s construction, not an intention Aja explicitly stated in the cited interview.

What does the final image of Nolan mean?
Nolan’s last moments leave open whether he has been affected by a supernatural force or whether the film is continuing its psychological uncertainty. The image can make possession feel possible without proving it. The story withholds the kind of independent, unambiguous evidence that would settle the question.
That uncertainty is the point viewers must carry out of the ending: Momma’s death does not resolve whether she was protecting her children from an external evil, acting under the power of fear, or caught in a reality where both explanations overlap.
Where can you check the film’s content guidance?
The UK classification record from the BBFC flags strong horror, violence, injury detail, suicide, and domestic abuse. This guidance applies to the BBFC’s UK classification record; it lists both a cinema version and a home-entertainment version, with the latter classified on September 30, 2024.
