Actor Lili Reinhart says she was diagnosed with endometriosis after months of urinary symptoms, repeated hospital visits and consultations with multiple doctors. In a first-person essay published by ELLE on March 12, 2026, she recounts that imaging identified adenomyosis but did not detect endometriosis; surgery on December 4, 2025, later found endometriosis. Her account describes one person’s diagnostic experience, not a diagnostic path every patient must follow.
What Lili Reinhart says happened
In her first-person essay for ELLE, Reinhart says urinary urgency and frequency began in summer 2024 while she was filming in Cologne, Germany. She describes going to hospitals, taking antibiotics, receiving a negative urine culture, and seeing urology and urogynecology specialists. She says she was treated for suspected interstitial cystitis.
Reinhart writes that roughly eight doctors had seen her without raising endometriosis before a pelvic floor therapist suggested it. According to her account, ultrasound and MRI identified adenomyosis, while the reports said, “No endometriosis detected.” An endometriosis surgeon later recommended laparoscopy to investigate and excision if lesions were found.
Reinhart says she underwent surgery on December 4, 2025, and learned afterward that endometriosis had been found. She reports leaving with diagnoses of endometriosis and adenomyosis. She also says an unexpected cyst and two hernias were removed and that her appendix was removed. These details are her account; they have not been independently reviewed here.

She describes the process as affecting her work, travel and anxiety, and says she felt dismissed. She also writes that she remains uncertain about fertility and future bladder pain. Her essay recounts messages from readers and a friend’s later diagnosis, but those are personal reports, not evidence of population-level effects.
What endometriosis is—and symptoms it can cause
Endometriosis is a condition in which tissue similar to the lining of the uterus grows elsewhere in the body. The NHS overview of endometriosis lists symptoms including severe period pain that disrupts daily activities, pelvic pain, pain when urinating or having a bowel movement, pain during or after sex, fatigue and possible difficulty becoming pregnant.

Urinary urgency and frequency were central to Reinhart’s experience, but those symptoms alone do not establish endometriosis. Symptoms vary and can overlap with other conditions, including adenomyosis, fibroids, pelvic inflammatory disease and irritable bowel syndrome. A clinician needs to assess symptoms in context.
How endometriosis can be diagnosed
There is not one test that provides every patient’s diagnosis. Assessment may combine a clinical history and examination with imaging; some patients may also be offered laparoscopy. Because symptoms overlap with other conditions, more than one step may be needed.

ACOG’s February 20, 2026 release says people wait an average of between four and 11 years from symptom onset to diagnosis. That is an ACOG-reported average, not a prediction of any individual’s timeline or a figure established for every country.
Can endometriosis cause pain when you pee?
Pain when urinating is among the symptoms listed by the NHS for endometriosis. Reinhart’s account centers on urgency and frequency, but neither urinary symptoms nor bladder pain alone can determine their cause. Persistent, recurring or disruptive symptoms are worth discussing with a healthcare professional, who can consider endometriosis as well as other possible explanations.

What treatment decisions may involve
Endometriosis care is individualized. The NHS describes pain medicines and hormonal medicines as common options, with surgery considered in some circumstances. NICE advises that decisions reflect a person’s symptoms, preferences and priorities rather than disease stage alone. Potential benefits, side effects, risks and fertility plans can all matter; no single treatment is right for everyone.
Reinhart’s account does not establish what treatment another person should choose. She wrote: “Women’s pain is not an inconvenience and should never be treated that way.”

Preparing for a clinical conversation
NICE says a pain and symptom diary can help discussions with clinicians. A brief record may include when symptoms occur, how they affect daily activities and any relevant cycle timing.
