Lewis Capaldi on Tourette Syndrome: What He Has Said

Lewis Capaldi has spoken about his Tourette syndrome diagnosis, shoulder twitch, treatment and the effect of a difficult 2023 performance on touring.

Lewis Capaldi has described Tourette syndrome as something he lives with, not as an explanation for every public moment or performance. Since disclosing his diagnosis in September 2022, he has spoken about a shoulder twitch, treatment he tried, the effect of symptoms and strain on performing, and the touring break he announced in 2023. In a 2025 interview, he looked back on that difficult period as a turning point.

When Lewis Capaldi disclosed his diagnosis

On September 7, 2022, Capaldi said in an Instagram Live that he had been diagnosed with Tourette syndrome. He said the diagnosis made sense in retrospect: watching older interviews, he could see his shoulder twitching. Before learning what was happening, he had worried that he might have a degenerative disease. He also said he decided to speak publicly in part because people had speculated that he was taking drugs. The Guardian reported his comments that day, as did ITV News.

How he has described his symptoms

Capaldi has described his tic as a shoulder twitch. In comments quoted by The Guardian, he said: “My shoulder twitches when I am excited, happy, nervous or stressed.” He also said the symptoms came and went, sometimes leaving him without them for months. “It is something I am living with,” he said.

In his Instagram Live, as quoted by ITV, he said: “I have Tourette’s. I’ve always had that apparently… so I do a shoulder twitch quite a lot.” ITV reported that Capaldi said some days were more painful than others, and that the twitch could look worse than it felt. These are his descriptions of his own experience, not a guide to how Tourette syndrome affects everyone.

What he has said about treatment and coping

Capaldi said he had received botulinum toxin injections, commonly called Botox, in his shoulder to reduce the twitches. He told ITV the treatment had worked for a time and also said he was learning ways to cope. Those remarks describe his own care; they are not treatment advice or a recommendation for anyone else.

Why his swearing should not be attributed to Tourette syndrome

The Guardian’s 2022 report explicitly said Capaldi’s profanity has nothing to do with his Tourette syndrome. His swearing should not be treated as a symptom or as evidence of what the condition is like for other people.

Glastonbury 2023 and his break from touring

At Glastonbury in June 2023, Capaldi struggled to continue “Someone You Loved” as his voice faltered and his shoulder twitched; the crowd sang along. On June 27, he announced a break from touring. In his statement, he said he was “still learning to adjust to the impact of my Tourette’s” and that the performance had made clear he needed time to address his mental and physical health. ITV News reported the announcement.

How Capaldi reflected on the episode in 2025

In a November 18, 2025 interview with NPR, Capaldi called the Glastonbury episode a “meltdown” and said: “I look at it now and I’m, like, that needed to happen.” He connected the period to pressure and overwork while making his second album, saying, “I’d really sort of worked myself into the ground making a second album,” and describing the expectation that he should match the success of his first. NPR reported that he returned to Glastonbury in June 2025 after nearly two years away.
