What an ADHD diagnosis at 40 means
An adult diagnosis does not mean ADHD first appeared in adulthood. ADHD symptoms begin in childhood and can continue through adult life. For an adult diagnosis, the National Institute of Mental Health says there must be evidence that symptoms were present before age 12. Clinicians may look back at childhood behavior even if no one recognized or assessed ADHD at the time. NIMH’s guide to ADHD in adults explains the age-of-onset requirement.

That distinction helps explain how someone can reach adulthood before receiving a diagnosis: symptoms may become easier to identify later, or an assessment may happen only after the person seeks help. The age of diagnosis is not the age of onset.
How clinicians assess ADHD in adults
There is no single test that establishes ADHD. The CDC says diagnosis involves multiple steps, and a healthcare provider may diagnose ADHD or refer someone to a mental health provider. Assessment typically considers symptoms and history alongside other possible explanations; sleep problems, anxiety, depression and learning disabilities can produce similar symptoms. - Childhood history: The clinician looks for evidence that symptoms were present before age 12. A person’s recollections may be supplemented by information from people who knew them as a child or by records such as school reports.
- Current symptoms and settings: Interviews, behavior rating scales or symptom checklists may help document current and past behavior. The clinician considers information from more than one source or setting where available.
- Other health factors: Mood and medical history are considered because other conditions can resemble or accompany ADHD symptoms.
Familiar experiences or an online quiz may prompt someone to discuss concerns with a healthcare provider, but they cannot establish a diagnosis by themselves.
Adult diagnosis is common, but access to care can be difficult
The CDC’s adult ADHD data page reports that more than half of adults with ADHD were diagnosed in adulthood. The figures summarize NCHS Rapid Surveys System data collected in the United States in October and November 2023; they are not measurements of 2026 prevalence. The same CDC summary says about one-third of adults with ADHD were receiving no type of treatment. Among adults taking stimulant medication, 71.5% reported difficulty filling prescriptions because of lack of availability. These figures describe different aspects of diagnosis, treatment and access—not the likelihood that any one person has ADHD. See the CDC data and its dates.
