Full manga spoilers for the Shibuya Incident and the final Sukuna battle. Nobara Kugisaki survived because Arata Nitta intervened after Mahito critically injured her, partially halting the worsening effects of Idle Transfiguration and leaving survival possible. She did not return immediately: after a long absence, she woke in chapter 267 and used Resonance on Sukuna’s remaining finger, disrupting him at a crucial moment and helping Yuji press his attack.
How Mahito injured Nobara—and why she survived
During the Shibuya Incident, Mahito touched Nobara and used his cursed technique, Idle Transfiguration. Her injuries were so severe that Yuji—and readers—could reasonably believe she had died.

Arata Nitta intervened and explained that his technique could partially stop the effects from worsening, preserving a chance that Nobara might survive. That intervention was not an instant cure or a resurrection. It bought her a possibility of survival after a devastating injury. The story does not provide a complete medical account of her recovery.
Why Nobara was absent for so long
Nobara does not rejoin the action soon after Nitta steps in. Her fate remains uncertain to Yuji and readers for a substantial stretch of the story. The account supported by the manga’s reveal is a sequence of survival being kept possible, an extended absence, and then an awakening—not a recovery shown in detail from day to day.

When Nobara comes back
Nobara returns in chapter 267, during the battle against Sukuna. Her first decisive move is to use the Straw Doll Technique’s Resonance on Sukuna’s remaining finger.
Because the attack lands during the fight, it disrupts Sukuna at a decisive moment and helps Yuji continue his assault. Nobara’s return therefore matters tactically: she does not simply reappear after recovering; she acts in time to affect the battle.

What the comeback does—and does not—explain
The confirmed explanation connects Nobara’s survival to Nitta’s intervention and her later awakening to the events of chapter 267. It does not establish a detailed treatment plan, an exact recovery timeline, or a fuller account of how she regained consciousness. Nor is there a verified author statement here that explains the creative intent behind the long delay; claims about that intent should be treated as interpretation, not fact.
