Fallout 4: Far Harbor never confirms whether Kasumi Nakano is a synth or a human. Kasumi believes she is a synth, but her memories and feelings are personal clues—not proof—and the faction records discussed in the story do not settle the question either.
What does Kasumi say about herself?
In her final holotape, Kasumi describes feeling out of place, gaps in her childhood memories, and dreams of waking in a laboratory. Those experiences lead her to question who she is and seek answers at Acadia. They explain why she suspects she is a synth; they do not independently establish her identity. The holotape transcript records her account.

Do the factions confirm whether she is a synth?
No. The available dialogue offers clues, but neither clue is a conclusive identity test.
- Railroad: A Railroad contact does not recognize Kasumi as someone the organization helped. That absence of recognition does not prove she is human.
- Institute: An Institute scientist reportedly finds no record of a synth deployed under Kasumi’s identity. The lack of a record is not definitive proof that she is human, particularly given the possibility of earlier memory manipulation.
These details are summarized in the Fallout Wiki character reference, a secondary source rather than an official ruling from Bethesda.

Why does Kasumi believe she is a synth?
Kasumi’s belief is rooted in her own experiences and her time at Acadia, a community of people who identify as synths. In dialogue transcribed by the Fallout Wiki, she says: “I… Yes it’s true… I mean, I can’t prove it, but when I made it to Acadia, I just knew…” The line communicates her conviction while explicitly acknowledging that she cannot prove it. The character reference transcribes the dialogue; it is not an out-of-game statement from the developers.
Does Far Harbor ever give a definitive answer?
No. Kasumi’s testimony tells players what she fears and believes; the faction clues fall short of verifying her identity. The game leaves room for players to interpret the evidence, rather than confirming that she is a synth or a human.

Where does Kasumi’s story take place?
Kasumi’s story is part of the Far Harbor add-on for Fallout 4. Bethesda says players can start the add-on during the main game by following the Valentine’s Detective Agency radio signal after “Getting A Clue,” or by finding the Nakano residence in the northeast of the Commonwealth. Bethesda Support explains how to get started with Far Harbor. Bethesda’s Far Harbor feature describes the add-on’s mystery-filled setting, but does not answer Kasumi’s identity question.
