The giant penis scene is in The Boys Season 3, Episode 1, “Payback.” Termite, played by Brett Geddes, shrinks and enters his partner’s urethra; after he sneezes and returns to full size, his partner is fatally injured. The sequence is fictional, and its premise is not medical guidance.
What happens in the Termite scene?
At a party, Termite uses his size-changing ability to shrink down and enter his partner’s urethra. He sneezes, involuntarily returns to normal size, and fatally injures his partner. The shock is typical of The Boys: an absurd superhero setup played as explicit gore. TheWrap recaps the scene in its interview with showrunner Eric Kripke.

How did they film it?
The production combined a large practical structure with visual effects; it was neither wholly practical nor wholly computer-generated. Kripke told the Los Angeles Times that the structure was 11 feet high and 20 feet long. Those are reported dimensions for this production prop, not a general statistic. The Los Angeles Times interview reports the measurements.
In a separate interview, Kripke estimated that the first view was roughly a 50/50 blend of practical work and CGI. He said the torso landing on the bed and much of the gore were CG. That proportion is his approximate account, not an independently audited visual-effects breakdown. As he put it to WIRED, “even though it was sweetened with VFX, it actually is a practical giant penis.”

Where did the idea come from?
Kripke said the writers’ room discussed an Ant-Man/Thanos meme and how its basic joke might work in The Boys. He credited episode writer Craig Rosenberg with bringing the scene idea forward. The sneeze gives the moment its sudden, involuntary turn from grotesque gag to fatal accident, according to Kripke’s WIRED interview.
