The studied course combined preparation sessions, supervised medication sessions lasting about eight hours, and psychotherapy afterward to help participants work with their experiences. The trial findings describe this research protocol; they are not instructions for self-treatment or proof that the approach is approved for routine PTSD care.
What “psychedelic-assisted therapy” means in this PTSD research
The phrase “psychedelic-assisted therapy” can suggest a single, standard treatment, but the term covers different substances and therapeutic approaches. The FDA materials relevant to PTSD here concern one investigational approach: MDMA given alongside a structured psychological intervention. They do not establish a general treatment method that can be assumed to apply to other substances or protocols.
That distinction matters when interpreting results. In the trials, participants received medication sessions within a planned course of psychotherapy. The evidence therefore concerns the combined treatment as studied, rather than MDMA tested independently of therapy or an ordinary psychotherapy visit with a drug added. The treatment was delivered in controlled clinical research settings, with screening and a defined schedule.
For a viewer trying to understand what happened in these studies, the simplest model is a sequence: preparation before medication, several supervised medication sessions, then integration sessions. The sequence is part of the intervention, not optional context around a pill. The FDA briefing describes the regimen used in the trials; it does not provide a basis for reproducing it outside clinical research.

What happened during the studied course
The FDA briefing describes psychotherapy before and after medication sessions, with the medication sessions separated by several weeks. There were three preparatory psychotherapy sessions before the first medication session. The treatment period then included three medication sessions, each approximately eight hours long. After each medication session, participants attended three integration sessions, making nine integration sessions across the course.
| Stage | What the FDA briefing describes | Purpose within the sequence |
|---|---|---|
| Preparation | Three psychotherapy sessions before the first medication session | Precedes the medication portion of the studied treatment |
| Medication sessions | Three sessions, each approximately eight hours, separated by several weeks | Supervised sessions within the psychotherapy program |
| Integration | Three sessions after each medication session; nine total | Psychotherapy following each medication session |
The first medication session used a lower total dose than later sessions in the trial protocol. The FDA briefing does not make this a general dosing recommendation, and the details above should not be used to obtain or administer MDMA. The important point for understanding the studies is that the intervention was a multistage clinical program, not a single appointment or a medication taken without therapeutic support.
“Supervised” also needs to be understood in context: this was a research protocol with screening, restrictions, and specified procedures. The briefing’s description is not a complete clinical guide to who can safely participate, how care should be delivered, or how to respond to an individual patient’s medical circumstances.

How researchers measured PTSD symptoms
The primary outcome in MAPP1 and MAPP2 was the change in total severity score on CAPS-5 from the start of the study to week 18. CAPS-5 is a clinician-administered, 20-item measure aligned with DSM-5 PTSD criteria. It is not a simple self-rating or a direct measure of how every participant will feel in daily life. Rather, it provided a consistent clinical measure for comparing symptom severity over the specified study period.
In its 2024 clinical review of the MAPP1 and MAPP2 trial data, the FDA reported statistically significant differences between the MDMA and placebo groups at week 18. The review’s table gives placebo-subtracted least-squares mean differences of −11.86 CAPS-5 points for MAPP1 and −8.51 points for MAPP2. The corresponding 95% confidence intervals were −17.41 to −6.32 for MAPP1 and −14.80 to −2.22 for MAPP2.
These are group-level estimates from the FDA’s review of the trials. They do not predict the change for a particular person, establish that every participant improved, or show that the same results would occur in a different treatment setting. A confidence interval expresses uncertainty around a study estimate; it is not an outcome range promised to individual patients.

What the trial results do—and do not—show
What the results support
The FDA review found statistically significant between-group differences on the primary PTSD symptom measure at week 18 in both MAPP1 and MAPP2. The figures are evidence about the studied MDMA-plus-psychotherapy protocols, the trial participants, and the assessment period. They give a more specific account of the results than a general claim that “psychedelic therapy works.”
What they cannot establish by themselves
- The effect of MDMA alone: The intervention paired MDMA with structured psychotherapy, so the results do not isolate a medication-only effect.
- Results for everyone with PTSD: Participants were selected for clinical trials, and the protocols included medical and medication-related screening. Trial participants are not automatically representative of every person who might seek treatment.
- Effectiveness in ordinary care: A controlled trial regimen is not equivalent to routine therapy. The trial setting, participant selection, therapeutic support, and treatment schedule all matter when applying findings beyond the studies.
- Long-term outcomes: The primary comparison described here was measured through week 18. That result alone does not answer how durable any changes are over a longer period.
- Unbiased participant expectations: The FDA briefing discusses concerns about adequacy of blinding and possible inadvertent bias. Those issues are relevant when judging how confidently trial results can be interpreted.
These limits do not erase the reported findings. They define what the findings can reasonably be used to say: the FDA review reported a statistically significant difference on a clinician-administered PTSD measure in two trials of a combined treatment program, while questions about blinding, the controlled setting, and how well the results carry over remain material.

How screening and restrictions fit into the protocol
Participation was not simply a matter of having PTSD and choosing to take part. The FDA briefing lists medical exclusions and medication-related restrictions used in the studies. Examples include uncontrolled essential hypertension, certain histories of arrhythmia, symptomatic liver disease or significant elevations in liver enzymes, recent ketamine use, and restrictions on psychoactive medicines.

These examples show why the trial results cannot be translated into a do-it-yourself checklist. They are study-protocol rules, not a complete list of risks, a screening tool for the public, or individualized medical advice. A person should not decide that a treatment is safe or unsafe for them by matching their circumstances to a few criteria in a trial briefing.

The regulatory picture needs a date attached to it. In its June 2024 advisory committee briefing on an application for MDMA capsules combined with psychological intervention for PTSD, the FDA stated: “MDMA is not currently approved for use in any market and is considered a new chemical entity.” That sentence describes the status documented in that 2024 briefing; it should not be presented as a statement of the agency’s status on a later date.
FDA materials updated in October 2026 describe continued research and development activity in psychedelic drugs, and the agency issued final general guidance for psychedelic clinical investigations in July 2026. Those materials indicate ongoing development work, but they do not, by themselves, establish that MDMA-assisted therapy is currently approved for PTSD. The 2024 briefing documents the application and status at that time; the later development materials are not an approval announcement. Anyone making a present-day decision about regulatory status should check current FDA product and application records.

Approval is also distinct from evidence that a treatment has been studied. A clinical trial can report results without making the tested intervention an approved routine-care treatment. Conversely, ongoing FDA guidance or development activity does not settle the disposition of a particular product application.
How to interpret a documentary or news account
When a report describes MDMA-assisted therapy for PTSD, a few questions help separate the trial evidence from broader claims:
- Does it identify MDMA specifically, or use “psychedelic therapy” as though every substance and protocol were the same?
- Does it make clear that the intervention combined medication sessions with preparation and integration psychotherapy?
- Are outcome figures attributed to the FDA’s review of MAPP1 and MAPP2 and identified as group-level CAPS-5 results at week 18?
- Does it distinguish a trial finding from routine-care effectiveness, an individual prediction, or regulatory approval?
- Does it acknowledge screening and the FDA briefing’s concerns about blinding and potential inadvertent bias?
These distinctions are especially important when an account moves from a measured clinical outcome to a broad promise about what a person with PTSD should expect. The reviewed trials provide evidence about a specific protocol and time point; they do not establish a universal response or a home-use method.
The evidence in one view
The FDA’s review of MAPP1 and MAPP2 describes a defined course of psychotherapy paired with investigational MDMA and reports statistically significant week-18 differences on CAPS-5 versus placebo. The figures are meaningful as trial results, but their interpretation depends on the clinical research setting, who was enrolled, the combined nature of the intervention, and the limitations FDA discussed. The regulatory record must be treated separately from the trial outcomes: the June 2024 briefing documented no approval at that time, while later FDA development materials do not establish present approval for PTSD.
