Spoilers for Grey’s Anatomy Season 22, Episode 1, “Only the Strong Survive.” Link survives the blast, though he suffers serious injuries and faces a long recovery. Dr. Monica Beltran dies after being trapped in the operating room, where she coaches Jules Millin through saving a young patient.
Does Link die in the Season 22 premiere?
No. Link survives the explosion and the medical crisis that follows, but he is badly hurt. A steel beam crushes him, and a piece of ceiling becomes lodged near his clavicle. The debris appears to limit severe bleeding, but the hospital’s damaged facilities leave the doctors with few options.

They improvise treatment in the CCU. While talking with Jo, Link goes into cardiac arrest and is resuscitated. Owen later operates on him, finding the source of the bleeding, and Teddy repairs it. Link has a lung laceration and other injuries. He reunites with Jo before the episode ends, but recovery will take time; actor Chris Carmack described Link’s injuries as “pretty extensive” in TVLine’s premiere coverage.
Who dies in “Only the Strong Survive”?
Dr. Monica Beltran
Beltran dies from injuries sustained in the blast. She and Jules are operating on six-year-old Parker when the explosion traps them. A surgical boom pins Beltran and crushes her pelvis. Despite her own injuries, she guides Jules through an airway emergency and Jules’s first solo surgery. Jules completes the operation and saves Parker, then discovers Beltran has no pulse.

The anesthesiologist
An anesthesiologist in the operating room is also dead by the time Jules and Beltran assess the scene. Beltran is the principal named physician death and the episode’s central loss; the anesthesiologist’s death is a separate detail.
What else happens after the explosion?
- Bailey, Lucas Adams, Jules, and Richard Webber are physically okay early in the episode; Webber has a head laceration.
- Lucas and Benson “Blue” Kwan manage Nora Young’s dangerous transfer and keep her alive.
- Simone tells Lucas she slept with someone else and ends their renewed relationship.
- Teddy tells Owen she no longer knows who she is, and Owen leaves for a hotel.
- New intern Dani Spencer is injured and needs surgery. Meredith helps treat her outside the damaged operating rooms.
- Ben Warren supports Owen’s decision to operate on Link, and Teddy reinstates Ben to Grey Sloan’s residency program.
What does the damage mean for Grey Sloan?
Meredith and Webber realize the hospital’s operating rooms will be unusable for weeks. Meredith’s response is, “We’re gonna rebuild.” That damaged infrastructure leaves the staff facing an extended recovery of the hospital as well as the patients caught in the blast.
