Spoilers ahead for Grey’s Anatomy Season 21, Episode 3, “I Can See Clearly Now.” Catherine survives a life-threatening complication, but Meredith’s decision to keep Richard in the dark leaves their friendship strained. Meanwhile, Bailey regains her residency-director role, Ben returns to training, and several residents confront difficult choices at work and at home.
What happens to Catherine, and why is Richard angry with Meredith?
Meredith and Bailey are performing a liver biopsy on Catherine when Catherine begins vomiting blood. They move to an emergency TIPS procedure—the intervention Meredith had wanted to perform. Richard is called away from another surgery and learns only then that his wife is in danger. Meredith and Bailey had kept the situation from him at Catherine’s request.
Catherine survives, but Richard later confronts Meredith. He argues that Catherine’s fear was a reason to tell him, not a reason to keep him uninformed. After the surgery, Catherine restores Bailey as director of the residency program.
Does Ben return to residency?
Sydney Heron interviews Ben about returning to residency and challenges his history of changing jobs. Ben makes the case that his varied experience has strengthened his commitment to medicine. Sydney hires him, in part to avoid putting Bailey in a position where her decision could be seen as nepotism.

How does the awake kidney transplant affect Simone and Lucas?
Nick Marsh arranges an awake kidney transplant for Tom, a patient who is also a scrub nurse. Richard assists with the operation. Simone had prepared for the case, but Sydney assigns Lucas to Nick’s service, and Simone joins the team anyway.
A missing sponge briefly alarms the operating team. The assignment also sharpens the friction between the interns: Simone accuses Lucas of benefiting from the Shepherd name. They later apologize. Nick encourages Lucas to treat repeating his intern year as an opportunity to become a better doctor.
What does Amelia’s tumor case mean for her next step?
Teddy brings Amelia a high-risk case: Rhiannon is 20 weeks pregnant and has a dangerous spinal tumor. Doctors have advised ending the pregnancy so the tumor can be treated more safely. Rhiannon is carrying a child for her brother and his husband and does not want to terminate.
Amelia agrees to operate while making clear that Rhiannon’s life comes first. The operation succeeds for both Rhiannon and the fetus. Teddy then tells Amelia that at least 15 patients with difficult tumors want her care, suggesting a new direction for Amelia: taking on cases other doctors have declined.

What changes for Mika, Jules, Levi, and James?
Mika learns that her 22-year-old sister Chloe has colon cancer. Although Mika and Jules have been growing closer, Mika ends the possibility of a relationship while she focuses on her sister.
In another patient story, Levi and Jules ask chaplain James to help Donna Mae, who has congestive heart failure, talk through a DNR. Donna had refused to sign until she learned that her ex-husband had died; afterward, she signs. Levi and James grow closer during the episode.
