Full spoilers: In the Season 20 finale, Meredith Grey refuses Catherine Fox’s ultimatum and publishes the Alzheimer’s research Catherine wanted handed to Tom Koracick. Teddy returns to help save firefighter Theo Ruiz, Jo discovers she is pregnant, and several careers and relationships are left at turning points.
What happens in “Burn It Down”?
Wildfires threaten the Seattle area, bringing a surge of patients to Grey Sloan Memorial. The hospital’s crisis unfolds alongside a confrontation over Meredith Grey and Amelia Shepherd’s Alzheimer’s research. Catherine Fox learns they continued the work despite her opposition—and that Teddy Altman secretly used hospital funds to support it.
Catherine gives Meredith and Amelia an ultimatum: turn over their findings so Tom Koracick can continue the research, and they, along with Teddy, can keep their jobs. Meredith refuses. Rather than hand the work to Catherine’s chosen successor, she publishes the initial results online and resigns from the Catherine Fox Foundation.
Does Teddy return to surgery, and who gets fired?
Teddy operates on Theo Ruiz
Teddy has been removed as chief of surgery, but she returns to Grey Sloan to help during the wildfire emergency. She, Owen Hunt, and Miranda Bailey treat Theo Ruiz, a firefighter and Station 19 character injured during that show’s penultimate episode.

Lucas’s exam result sparks a standoff
The interns and residents pass their exams, and Lucas earns the highest score. That strong result does not settle whether he can advance: Catherine is poised to decide if he must repeat the year.
