Spoilers for “Wish World” and “The Reality War.” In the two-part finale—called “Season 2” on the official Doctor Who site, though some viewers and platforms use “Season 15”—Ruby ends Conrad’s wish and Omega is defeated, but the restored reality leaves Poppy erased from ordinary memory. The Doctor risks his life to bring her back, then regenerates. Billie Piper appears afterward, but the episode leaves her identity and role unanswered.
What happens at the end of Doctor Who Season 15?
The finale’s immediate threats are resolved, but restoring Poppy’s life comes at a cost to the Doctor.
- Omega attacks. The Rani plans to use Omega’s body as a gene bank to create a new race of Time Lords. Omega emerges from the Underverse as a decaying titan, calls himself a god of time and eats the Rani. The Doctor uses the stolen Vindicator to blast Omega back into the Underverse. The official episode synopsis recounts the confrontation.
- Ruby ends Conrad’s wish. The altered reality changes permanently: Conrad survives, but has no memory or record of his former identity. Poppy disappears, and Ruby is the only person who remembers her.
- The Doctor intervenes to save Poppy. He leaves his companions and tries to rescue her. His Thirteenth incarnation appears in the TARDIS because the act is about to create a “giant time schism.” The Doctor releases regeneration energy into the console, creating that schism.
- Reality realigns, then the Doctor regenerates. Poppy is safe and remains Belinda’s daughter, now with a human father rather than the Doctor. The Doctor says goodbye to Belinda and Poppy; afterward, the Fifteenth Doctor regenerates.
Why does Poppy disappear—and how does the Doctor save her?
Ruby’s decision to end Conrad’s wish removes the altered reality that had sustained Poppy’s place in it. The change is not a simple rewind: Poppy vanishes from ordinary memory, while Ruby alone remembers her. The episode’s official synopsis describes these immediate consequences, but does not spell out every continuity change as a comprehensive reset.
The Doctor’s answer is to risk himself. He uses regeneration energy to create the time schism, and reality realigns so Poppy survives as Belinda’s daughter. The ending preserves their family while changing the Doctor’s connection to it: he is Belinda’s “Uncle Doctor,” and Poppy has a human father. The emotional resolution is therefore not just that the universe is safe; it is the Doctor choosing to save a child even when doing so may cost him his life. That reading follows the decision and its consequences shown in the finale.

Did Ncuti Gatwa’s Doctor regenerate?
Yes. The Fifteenth Doctor regenerates after saying farewell to Belinda and Poppy. The official post-finale announcement identifies this as Ncuti Gatwa’s final episode and describes the Doctor’s action as the ultimate sacrifice to save a little girl. The regeneration is part of the story, not merely a visual hint that Gatwa is leaving.
Who is Billie Piper after the regeneration?
The episode does not identify Piper’s character. The official announcement presents her as a familiar returning face but says that “just how and why she is back remains to be seen.” Russell T Davies likewise said, “quite how and why and who is a story yet to be told.” An Associated Press report after the broadcast also noted that her role was being kept secret.

So the ending does not establish that Piper is Rose Tyler, nor does it confirm that she is the next Doctor. Those are possibilities viewers may speculate about, not answers supplied by the finale or the cited announcements.
What does Omega’s return mean for Doctor Who continuity?
The official show site describes Omega as the “first Time Lord” and says he was last seen in “Arc of Infinity” (1983). In this finale, he has spent time trapped in the Underverse before emerging as a godlike creature. His defeat sends him back there; the Rani is killed by Omega.

Davies has framed this version of Omega as a reinterpretation, not a definitive explanation of every part of Gallifreyan history. In an official interview published 19 June 2025, he said: “I used the Time War to tweak the legend of Omega. A lot of things said about Omega were not said in the original series. You have to accept that 40 years have passed. The Time War came along and everyone’s history got changed. So that’s what Omega is now. You can reinvent it.” His explanation treats continuity as something the show can recontextualize over time; it does not make the finale an exhaustive account of Gallifreyan history.

The numbering differs by source. The official Doctor Who site labels the 2025 finale “Season 2”; “Season 15” is an alternate numbering used by some viewers and platforms. The two-part story is “Wish World,” followed by “The Reality War.” The original release announcement names Russell T Davies as writer and Alex Sanjiv Pillai as director and gives 31 May 2025 as the original release date.
