Keaton played Kay Adams, who marries Pacino’s Michael Corleone in the film. That fictional marriage became the pair’s best-known shared story, but it was separate from their real-life relationship.

Keaton later recalled that she was 23 during the first film’s production, felt like an outsider, and did not know Pacino well at the time. In a 1997 interview with Terry Gross, she put it simply: “I think in every way, I felt the outsider.” Her recollection complicates the assumption that the actors were already close while making the original film.
After working together: Attraction and an on-and-off relationship
Keaton later said she had a crush on Pacino, whom she remembered as charming, funny, talkative, and gorgeous. Retrospective accounts describe their romance as on and off, but they do not pin down when it began.

In a 2017 interview, Keaton described the relationship as lasting five years. The Guardian, in its account of the couple, says it continued into the 1980s. These are differently framed recollections, not a precise, continuous date range.
Marriage hopes and the breakup
Keaton said she thought Pacino might eventually marry her, though she did not want a formal proposal. “I didn’t even want him to propose. I just thought maybe he would marry me, eventually,” she said in the 2017 interview. She added that not marrying was “a blessing for both of us,” saying it would have been “a nightmare for him.”
According to Keaton’s recollection as reported by The Guardian, she gave Pacino an ultimatum because he would not commit to marriage, and the relationship ended. She also said the breakup was not her choice. The accounts do not establish the year of the ultimatum or the final separation.
Looking back, Keaton described a mismatch in what each needed from a partner: “We’re very eccentric, he needed a woman that was going to take care of him, I needed a man who would take care of me … It was just very important that we left each other alone, said goodbye,” she said in the same 2017 interview.

In a tribute published after Keaton’s death, Pacino called her his partner and friend. He said: “Diane was my partner, my friend, someone who brought me happiness and, on more than one occasion influenced the direction of my life.”
He also reflected on the time since their relationship: “Though over thirty years has past since we were together, the memories remain vivid, and with her passing, they have returned with a force that is both painful and moving.” Of Keaton, he added: “She lived without limits, and everything she touched carried her unmistakable energy.”
The films and the relationship are different stories
As Kay and Michael, Keaton and Pacino portrayed a marriage shaped by the Corleone family’s power and violence. That onscreen relationship is not a record of their private life. Keaton’s account that she felt outside the group and did not know Pacino well during the first film is a reminder that the cinematic story and their later romance had different timelines.
