What happens in “Sink or Swim”
The premiere’s central emergency begins with two ships colliding near Navy Pier. The resulting mass-casualty influx puts the hospital under pressure and gives the episode a single, urgent frame for its many storylines.

That frame also carries the transition into a new season. The episode addresses the preceding cliffhanger and makes room for cast and character changes while the staff responds to the crisis. Dr. Caitlin Lenox, played by Sarah Ramos, and Dr. John Frost, played by Darren Barnet, are among the new doctors introduced in the premiere context. Dr. Crockett Marcel’s exit and changes involving Dr. Daniel Charles and Liliana add further movement.
Does the premiere feel too crowded?
That is the episode’s central trade-off. The ship emergency gives the premiere momentum and a practical reason to bring multiple characters and changes together. But with the crisis, cliffhanger resolution, new arrivals and departures all competing for attention, the hour has a lot to establish at once.

CBR reviewer Brittany Frederick described it as “an episode so big that it actually gets hard to embrace.” That is a reviewer’s judgment, not an official assessment. Frederick credits new showrunner Allen MacDonald with juggling the chaos, while arguing that the episode’s ambition backfires in places. The mixed verdict fits the structure: the premiere is capable at managing a large ensemble, yet its volume of concurrent developments can make the transitions harder to absorb.
What the premiere sets up
“Sink or Swim” signals that Season 10 will begin with change as well as crisis. Lenox and Frost enter the hospital’s orbit, while Marcel’s exit and developments involving Charles and Liliana give returning viewers several character threads to track. These are premiere-level changes; they do not, on their own, establish how every character’s story develops later in the season.

For viewers deciding whether the episode works, the useful question is less whether it is busy than whether the emergency makes its many transitions feel connected. It provides a common setting and pressure, but the number of developments leaves less room for each one to settle.
