Review: the episode’s strongest thread is Lenox learning to mentor
Lara Rosales’s Tell-Tale TV review describes an unusually graphic emergency in which a patient’s lung “pops out” of his chest. The medical crisis is more than a shock beat in her reading: it forces Lenox to face the responsibilities and feelings that come with mentoring Naomi Howard.

Lenox pushes Naomi beyond her comfort zone, but has trouble acknowledging how invested she is when Naomi applies what she has learned. Archer’s advice helps Lenox recognize that she can be proud of Naomi’s work. That tension gives the mentorship thread its shape: effective guidance means taking a risk on someone else’s judgment, then resisting the urge to treat their growth as a threat to your own control.
Apple TV’s terse synopsis captures the premise—“Lenox’s mentorship is put to the test”—but Rosales’s review supplies the emotional contrast. The episode’s title suits this strand: Lenox has to examine not only Naomi’s choices, but her own reflexes as a teacher.

Ripley’s remorse does not excuse the danger he creates
Ripley’s storyline is less a redemption arc than a warning about guilt overruling judgment. According to TV Insider’s recap, the man Ripley fought arrives at the emergency department in serious condition, and Frost is treating him. Ripley favors a less invasive approach and pulls rank. When that plan fails and the team must open the patient’s chest and cross-clamp the aorta in the ED, Ripley admits he injured the man. TV Insider reports that the patient is expected to recover.
Rosales reads Ripley’s continued involvement in the patient’s care as a return to behavior associated with his troubled youth: remorse keeps him close, but it does not make him safe to manage the case. Goodwin suspends him, and TV Insider specifies that the suspension is without pay while a formal investigation, hearing and judgment proceed. Goodwin says she will recommend termination; that is a proposed recommendation, not a reported final board decision.

The episode’s central distinction is between feeling sorry and acting responsibly. Ripley’s guilt may be sincere, but it does not undo the conflict of interest or the risk created when he favors his own judgment over the team’s care plan. Frost’s reaction makes that breach legible. Actor Darren Barnet, who plays Frost, told TV Insider: “Frost is kind of getting to know the dark side of Ripley, but he’s definitely not liking it. He’s a bit jarred by it because this man is very beaten up. It wasn’t just a fight. Ripley really, really, really went in on this guy.” Barnet also summarized Frost’s view of a physician’s duty: “You are a physician. Your job is to help and cure and heal and help people recover, not put them in the hospital.” These are Barnet’s comments about the characters, not dialogue from the episode.

Charles and Jackie: a boundary problem the review does not resolve
Rosales raises the episode’s most troubling professional-boundary question through Charles and Jackie. Her review says Charles had been Jackie’s doctor when she was admitted to a psychiatric ward, later testifies in her custody trial, and becomes romantically involved with her. Rosales characterizes his testimony as perjury; that is the reviewer’s account and legal characterization, not an independently established legal finding.
The sequence makes the relationship difficult to read as an ordinary romance. A prior doctor-patient connection carries an imbalance that does not disappear simply because the clinical encounter has ended. The episode places Charles’s personal interest alongside that history, but the review’s concern is sharper than a simple question of whether the pair have chemistry: it is whether a doctor can cleanly separate an intimate relationship from the authority and trust built through care. The material described in the review raises that issue without resolving it.

Maggie’s patient story adds a quieter question of recognition
Rosales also describes Maggie helping a transgender woman with dementia who had stopped receiving hormone replacement treatment. In the review’s account, Maggie helps her present in a way that reflects how she feels, and a new LGBTQ+ memory center is part of the storyline. This detail is not included in the short Apple TV synopsis, so it should be understood as plot information reported by the reviewer.
The thread shifts the episode’s focus from authority and accountability to recognition: care includes seeing the person in front of you, not reducing her to a diagnosis. It is a fictional scenario, not clinical guidance about dementia, hormone treatment or individual care decisions.
Verdict
“Take a Look in the Mirror” is most compelling when it asks how doctors handle the personal stakes attached to professional roles. Lenox’s mentorship shows the vulnerability involved in trusting a colleague to grow; Ripley demonstrates how remorse can become dangerous when it displaces sound judgment; and Charles’s relationship with Jackie exposes the lasting weight of a doctor-patient history. The episode’s graphic emergency and several dense plotlines may compete for attention, but its strongest ideas converge on the same point: good intentions do not erase the consequences of crossing a boundary.
