After Firehouse 51 responds to a weekend fire at a school, Severide and Van Meter investigate signs that it was deliberately set. Their inquiry centers on Principal Cole, who appears to have been the target, and on a student group hostile to him.

A smart plug used to ignite the school fire remotely complicates the early suspicion of a student who had an alibi. The hostile group broadens the list of possible suspects, but the episode does not establish who started the school fire.
Severide and Van Meter visit Cole at his apartment to discuss the case. In the closing sequence, a hooded figure pours gasoline around the building’s entry-level area and sets the stairwell alight. The three men are left upstairs as the fire burns below them. The episode cuts before showing an escape, rescue, or death.

Does Severide die in the Season 14 midseason finale?
No death is confirmed. At the episode’s cutoff, Severide’s fate—and Van Meter’s and Cole’s—is unresolved. The final scene establishes immediate danger, not the outcome.
Who set the fire at Cole’s building?
The hooded person is not identified in “Pierce the Vein.” The school investigation offers clues and possible motives, but neither the smart plug nor the student group proves who is responsible. The episode leaves the attacker’s identity open.
Why is Mouch’s Engine 51 at risk?
Annette tells Pascal that the city is reallocating funds to avert a teachers’ strike and has asked the fire department to identify rigs for permanent decommissioning. Mouch’s Engine 51 is on the proposed list. Pascal learns about the threat, but the finale does not say that Engine 51 has been shut down or that its future is settled.
This budget cliffhanger runs alongside the apartment fire: one threatens three people’s immediate safety, while the other puts a key piece of Firehouse 51’s equipment at risk. Pascal’s personal ties to the firehouse add pressure to his position as the department faces a budget decision.
