Spoilers ahead. Manel and Viktor do not die in the hospital: they escape by helicopter with the surviving group. But the film ends on a warning from Manel’s sister, Belén, telling him not to come to the Canary Islands. It never reveals exactly what danger awaits there, or whether Belén’s family is alive.
What happens at the end of Apocalypse Z?
At the hospital, Manel and his companions try to reach a helicopter while contending with the infected and the armed group led by Ushakov. Sor Cecilia is bitten and left behind. Viktor gets the helicopter running, and the survivors escape. Manel hopes to reach the Canary Islands and reunite with his sister, Belén.

That escape resolves the immediate struggle at the hospital, but not the group’s search for safety. The film’s final turn comes when Belén warns Manel not to come to the place where she is staying.
Do Manel and Viktor die?
No. The film shows Manel, Viktor and the surviving companions escaping the hospital by helicopter. It does not show them dying afterward. Belén’s warning makes their next destination uncertain, but it is not a confirmation that either man is dead.

What does Belén’s warning mean?
The warning establishes that the place Manel expected to be a refuge may be dangerous. It does not explain what has happened in the Canary Islands, why Belén tells him to stay away, or whether her family is alive. The film does not confirm that the islands have fallen or identify a specific threat there.
The ending is best read as a cliffhanger: Manel has escaped one crisis, but the reunion and safety he was traveling toward remain out of reach. That is an interpretation of the events onscreen, not a confirmed explanation from director Carles Torrens.

How the film’s ending differs from the novel
The film adapts Manel Loureiro’s novel of the same title, but the novel should not be used to fill in answers the movie leaves open. El Comercio reports that the hospital children were added for the film and that Cecilia’s death also differs from the source. Loureiro described the adaptation’s approach this way: “Necesitábamos que esta película tuviese una mayor potencia visual e inmediata y eso se producía con algo más coral.” In English: “We needed this film to have greater visual and immediate impact, and that came through something more ensemble-oriented.” The English wording here is a translation of the Spanish quote reported by El Comercio.

Amazon has announced Apocalypse Z: New World for worldwide Prime Video release on October 30, 2026. The announcement places its story in the Canary Islands, described as Europe’s last refuge after the outbreak. That setting makes the sequel relevant to Belén’s warning, but the announcement does not promise that it will answer every question the first film leaves open.
