They may strike you as lookalikes in a particular photo or role—but “long-lost identical twins” is a playful exaggeration, not an established fact. A resemblance is a personal impression, and noticing one is different from mistaking one actor for the other.
Why might they look alike?
Bruce Willis and Kevin Costner have both spent decades on screen, so the image you have in mind matters: their appearance varies across roles, hairstyles, ages, lighting and camera angles. Compare specific stills rather than treating “look alike” as a fixed verdict. You might ask whether the resemblance comes from face shape, a particular expression, or simply the look of two leading men in a certain era.
That comparison is subjective. Research on faces distinguishes judging whether two faces seem similar from recognizing who a person is; people can see a resemblance without believing the people are identical. The paper also notes that observers may disagree about similarity: research on perceived facial similarity and face recognition.
Which roles are you picturing?
Familiar screen images can shape the comparison. Costner is associated with Field of Dreams, Dances with Wolves, The Untouchables and Yellowstone; Willis with Die Hard, Pulp Fiction, Armageddon and The Sixth Sense. Those roles offer very different ages, styling and expressions to compare, rather than one definitive pair of faces. Do people regularly mix them up?
There is no basis here for saying audiences generally confuse the two, and no reviewed source directly measures how similar their faces are. One commenter recalled mixing up Willis, Costner and Mel Gibson as a child—a personal memory, not evidence of a broader pattern. If you see the resemblance, it may be tied to a particular image or moment rather than a reliable likeness across their careers.

Put two specific images side by side and say what prompted the comparison: face shape, hairstyle, age, expression, role, lighting or camera angle. That makes “they look alike” a more useful conversation than an unsupported claim that everyone sees the same thing.
