Spoilers for the full film. Rain Carradine and Andy are the two characters who survive the ending of Alien: Romulus. Rain ejects Kay’s rapidly grown hybrid child, the Offspring, into space, then sets course for Yvaga with the badly damaged Andy in cryosleep. The film does not show whether they reach the planet.
What happens at the end of Alien: Romulus?
The derelict Renaissance station holds the remains of a Weyland-Yutani experiment. Rook explains that the company is trying to adapt material derived from the Xenomorph into a compound it hopes could improve humanity’s prospects in space. The survivors take vials and attempt to escape. The official 20th Century Studios film page describes young colonizers scavenging a derelict station before encountering its hostile life form.

Kay is injured and later injects herself with the compound in an effort to survive. Rain and Andy make it back to the Corbelan IV, but Kay’s pregnancy accelerates. She gives birth to a hybrid creature known as the Offspring, which attacks Andy and kills Kay. In the cargo area, Rain releases the cargo hold and ejects the creature into space. She returns to the ship and sets course for Yvaga, placing the damaged Andy in cryosleep and promising to repair him.

What is confirmed, and what remains unresolved?
Rain and Andy escape aboard the Corbelan IV. Their eventual arrival on Yvaga is not shown, and the film does not establish whether Rain can repair Andy. Any claim about their later fate or a sequel’s story goes beyond what this ending confirms.
Rain ejects the Offspring from the ship, ending its immediate threat to her. The ending does not develop what ultimately happens to the creature after that moment. Kay’s injection is not presented as a successful cure: she dies after giving birth to the hybrid.

What does the ending suggest about the compound?
The plot shows Weyland-Yutani trying to turn alien biology into a tool for human survival, Kay using the compound in hope of surviving, and that effort culminating in the Offspring’s birth. One reading is that the experiment’s promise of progress is bound up with exploitation and body horror. That is an interpretation, not a single definitive moral stated by the film.
Director and co-writer Fede Álvarez told D23 that “The whole point of this movie was to take back Alien to the simplicity of the beginning of the franchise, where it was, above all things, a horror movie.” The D23 interview attributes the statement to Álvarez and identifies him as the film’s writer-director.
