Adriana Smith, an Atlanta nurse who was about two months pregnant, was declared brain dead on February 19, 2025. Her family said clinicians told them Georgia’s abortion law required continued life support; Georgia’s attorney general said the law did not require it after brain death. Smith’s son was delivered prematurely in June. Emory Healthcare did not publicly explain the legal reasoning behind its case-specific care.
What happened to Adriana Smith
Smith, 30, suffered a medical emergency involving blood clots in her brain and was declared brain dead on February 19, 2025. Her family said she was about two months pregnant. Early reports described her as roughly eight or nine weeks pregnant, but accounts differ, so a more exact gestational age is not established.
After the declaration, clinicians continued measures that supported Smith’s bodily functions while the pregnancy continued. The hospital did not publicly detail which measures were used or disclose a complete, case-specific care plan. Brain death is legal death: ventilation and other organ-support measures can maintain bodily functions, but that does not mean the person is in a coma or may recover.
Why the explanation for continued support is disputed
The public record contains three different accounts, and they do not add up to a settled explanation of the decision.

Public accounts of the family’s wishes and the hospital’s reasoning remain incomplete and, in places, conflicting. A January 2026 article in the Georgetown Journal of Gender and the Law noted that public information had not established whether Smith had an advance directive. The available reporting also does not establish the full clinical timeline before her brain-death declaration or what legal advice the hospital received.
What Georgia’s abortion law says—and what it does not settle
Georgia’s LIFE Act was enacted in 2019 and took effect after the U.S. Supreme Court’s 2022 decision in Dobbs v. Jackson Women’s Health Organization. The law generally prohibits abortion once fetal cardiac activity is detectable, often around six weeks of pregnancy. It includes exceptions, including when an abortion is necessary to preserve the pregnant patient’s life or physical health, and it contains fetal-personhood language.

The Associated Press reported that the law does not explicitly address the precise circumstance of a pregnant patient declared brain dead. That gap matters: the family’s account of what clinicians said and Carr’s public interpretation differ, while Emory’s general statement did not resolve the conflict. The available reporting does not establish that a court determined the hospital was legally required to continue support.
When Smith’s son was born
In June 2025, after roughly 16 weeks of support, Smith was taken for an emergency cesarean delivery. Georgia Public Broadcasting reported that Chance was born at about 25 to 26 weeks’ gestation and weighed 1 pound, 13 ounces. Smith was subsequently removed from life support.

In a report published about six months after the birth, ABC News quoted Smith’s mother as saying Chance remained in a neonatal intensive care unit and could not breathe independently. That is a family-reported update from late 2025, not a verified account of his condition in 2026.
How this case compares with reported medical outcomes
Pregnancy after brain death is medically complex, and published case reports are not a prediction of what will happen in an individual pregnancy. A 2021 literature review co-authored by maternal-fetal medicine specialist Vincenzo Berghella identified 35 known cases in which clinicians aimed to prolong pregnancy after brain death. As reported by the Associated Press and Georgia Public Broadcasting in 2025, 27 resulted in live births, and pregnancy was prolonged by an average of seven weeks before complications forced intervention.

Those figures describe a small collection of reported cases and the outcomes in that review. They do not establish Smith’s individual prognosis or explain the hospital’s decisions in her case.
