Near the end, Sam sends Eric toward Henri’s evacuation boat with Frodo. She makes noise to draw the creatures away from them. Eric and Frodo reach the boat. In the final sequence, Sam walks away with music playing; a creature appears behind her, and the film ends without showing what happens next.
Does Sam die?
The ending strongly implies that Sam is killed: she walks into danger alone, and a creature appears behind her. But the film does not show the creature attacking, a body, or an explicit confirmation of her death. The distinction matters: her death is the clear implication, not an on-screen event.

Her terminal illness also remains unchanged. The film offers no cure or new plan that would give her a long future, so her choice is made with the likelihood that she has little time left already.
Why does Sam choose to draw the creatures away?
Sam’s decision gives Eric and Frodo a chance to escape, but it is more than a conventional sacrifice for strangers. By helping someone she has come to care about and ensuring Frodo is with him, she shapes how she spends what she believes may be her final hours. The pizza and the place connected to her father are part of that same effort to reclaim moments that matter to her, even as the world collapses.

Writer-director Michael Sarnoski said he wanted to honor the fact that Sam is a dying person rather than sugarcoat it, while still allowing her to find agency and joy near the end of her life. He told GamesRadar+ the ending was intended to feel triumphant and leave audiences “life-affirmed.” That framing explains why the scene can be tragic in outcome yet affirming in what Sam chooses to do.
Do Sam and Eric survive?
Eric and Frodo make it aboard Henri’s boat. Sam walks away from the boat to pull the creatures toward herself, and her death is strongly implied but not shown. The film does not establish what happens to Eric or Frodo after they escape.

How does the ending connect to A Quiet Place Part II?
The boat links Day One to the island refuge in A Quiet Place Part II. Part II establishes that survivors reach the island by boat and that the creatures cannot swim. Day One shows Eric and Frodo escaping on a boat associated with Henri, but it does not confirm that either character appears in the sequel.
Sarnoski said he did not think Eric was secretly present in Part II; he joked that viewers could take any cat seen on the island to be Frodo. That is a playful possibility, not confirmation of a cameo or a canonical identification.
