Spoilers for Season 9, Episode 6, “Family History.” Hen collapses, but the episode does not reveal what is wrong with her. She hides the incident from Karen and seeks scans on her own, leaving her health as the central cliffhanger. Meanwhile, Chimney accepts the permanent captain job, and Harry gets into the fire academy.
What’s wrong with Hen at the end of “Family History”?
The episode leaves Hen’s medical problem undiagnosed. She has noticed a rash and a tremor in her hand. During a call at a medical spa, a doctor sees the tremor, but Hen distrusts him. Later, she calls her mother to ask about family medical history, then collapses at home and lies on the floor for hours before waking.
Hen conceals the collapse from Karen. After asking Maddie about symptoms and testing, she returns to the spa, pays out of pocket and asks for scans without telling her loved ones. The scene sets up a medical-mystery cliffhanger; it does not confirm cancer or any particular neurological condition.
Does Chimney become captain of the 118?
Yes. After serving as interim captain following Bobby’s death, Chimney reconsiders Chief Simpson’s offer and accepts the permanent position. At the ceremony, the crew gives him a watch engraved, “The 118 is not just a number, it’s us.” The inscription marks his place in the team rather than presenting him simply as Bobby’s replacement.

Harry is accepted into the fire academy
Buck helps Harry train and urges him not to give up on his application because of his record. Harry is accepted into the LAFD academy.
Athena’s call brings May’s past back into focus
Athena responds to a teenage girl’s overdose after cyberbullying. The case recalls May’s own overdose years earlier and gives Athena and May an opening to talk about what happened and May’s recovery.

Which Season 9 finale is “Family History”?
“Family History” is Season 9, Episode 6, the fall or midseason finale—not the season’s final episode. ABC lists the episode in its Season 9 episode guide. The later season finale is Episode 18, “Hearts and Flowers.” The midseason episode aired in November 2025; TVLine’s recap was published November 13, 2025, and TV Insider identified Episode 6 as the fall finale ahead of the show’s January 2026 return.
