For a scientifically minded outbreak thriller, start with Contagion; for a documentary rooted in a real pandemic, choose 76 Days. The films on this list range from laboratory procedurals and historical dramas to infection horror and stories of life after society collapses. “Best” here means a curated set of strong options, not an objective ranking—and an absorbing film is not necessarily a reliable guide to public-health practice.
How to choose an outbreak movie
Outbreak films do not all tell the same kind of story. Some follow scientists and public-health officials; others focus on the social costs of disease, the terror of infection, or a world already transformed by catastrophe. A quick way to choose is to consider three things:

- Scientific process: Look for stories about surveillance, laboratory investigation, containment, or vaccine distribution.
- Scale: The threat may be confined to a city, spread internationally, or form the backdrop to a post-apocalyptic society.
- Treatment: The disease may be fictional, speculative, tied to a historical epidemic, or part of a documentary about a real crisis.
One study in JAMA identified 373 U.S.-theatrical films released through December 31, 2019, with a major infectious-disease focus. Of those, 142 (38.1%) featured a human outbreak or pandemic as an important story element; the authors selected 80 as culturally relevant using their stated box-office, award, or long-gap criteria. These are counts from the study’s search and selection method, not an objective ranking of the best films. The paper defines an outbreak as an increase in expected cases in a population and a pandemic as an outbreak spanning multiple countries or continents. Read the JAMA study.
Scientific process and public-health response
Contagion
This international-spread thriller gives substantial attention to scientific investigation and the logistics of vaccine distribution. Its fictional MEV-1 is modeled on the coronavirus associated with SARS. A medical review notes a pacing limitation: the film moves quickly from vaccine development to mass production. The Andromeda Strain (1971)
A satellite crash near a New Mexico town brings an extraterrestrial organism to Earth, and a research team studies it in a secret laboratory. Robert Wise directs this science-fiction procedural, which makes investigation and containment central to its suspense. Panic in the Streets
This New Orleans-set film builds its crisis around a plague outbreak and the effort to respond before it spreads. The JAMA study includes it among films portraying infectious disease. Outbreak
A mainstream epidemic thriller for viewers who want the threat to escalate beyond a contained incident. Its appeal is momentum and crisis-scale suspense; treat it as dramatized fiction rather than a source for how an actual outbreak unfolds.

This historical drama offers a more intimate angle on epidemic disease and its consequences. Choose it for a relationship-centered story shaped by illness and place, rather than a laboratory race or action-driven containment plot.
Blindness
A speculative epidemic premise turns attention toward how people behave when ordinary social structures come under strain. It suits viewers more interested in social breakdown and moral choices than in scientific procedure.
Infection horror and survival
28 Days Later (2002; 2003 U.S. release)
Danny Boyle’s film follows Londoners searching for sanctuary after a Rage plague devastates the population. The official studio page identifies Alex Garland as the writer and lists an R rating and a 1-hour, 53-minute runtime. 28 Weeks Later (2007)
The sequel begins six months after the military declares Britain safe from the Rage virus, then sets a new crisis in motion. This infection-horror choice keeps the focus on survival and the decisions people make under threat. It fits a darker, character-centered watchlist rather than one built around laboratory work or a detailed institutional response.

The Omega Man
For a post-apocalyptic take, this story shifts attention from containing an outbreak to living in the society left behind. It is a fit for viewers who want survival and a transformed world more than the early stages of a crisis.
The Last Man on Earth
This classic post-apocalyptic option puts isolation and survival at the center. It belongs on an outbreak watchlist for its vision of life after catastrophe, rather than for procedural detail.

Also discussed in the JAMA study, this HIV-era drama centers the human and social consequences of the epidemic. It is not a conventional outbreak thriller, but belongs in a broader list about films shaped by pandemic-era illness and stigma.
A documentary about a real pandemic
76 Days (2020)
Unlike the fictional scenarios elsewhere on this list, this documentary-drama is set in four Wuhan hospitals during the COVID-19 crisis and focuses on patients and healthcare workers. Director Hao Wu’s film runs 93 minutes, according to Paramount. It is the clearest choice here if you want a documentary account of people inside a real crisis rather than an imagined outbreak. The distinction matters: a documentary records a specific crisis, while a fictional movie can compress timelines, invent pathogens, or heighten danger for dramatic effect. For instance, an Alpha Omega Alpha review discussing SARS reports 8,069 cases and 774 deaths as historical figures; those numbers are not figures for the fictional disease in Contagion or current disease data.
