This editorial ranking covers films released from 2021 through 2026, inclusive, as of October 7, 2026. The five-year window is a rolling one, not a standard calendar-year bracket. The available source material supports ten distinct recommendations rather than fifteen, so this list does not pad its final places with unverified picks.
“Psychological thriller” is a flexible label, not a formal genre rule. Here, suspense must depend chiefly on unstable perception, secrets, obsession, paranoia, moral uncertainty, or psychological pressure. The ranking weighs sustained tension, storytelling and reveal structure, performances, originality, and the uncertainty or moral tension left after the credits. It is an editorial judgment, not a universal consensus or a Rotten Tomatoes score ranking.
10 best psychological thrillers released from 2021 to 2026
1. Anatomy of a Fall (2023)
A novelist is accused of her husband’s death, and the legal case brings their private resentments and jealousies into public view. The film turns conflicting accounts and intimate details into evidence without making the central uncertainty easy to settle. Its courtroom structure gives the tension a precise framework, while the shifting boundary between what can be proved and what can be believed gives it lasting force.
Rotten Tomatoes placed it first in its 2023 mystery-and-thriller list and displayed a 96% Tomatometer score on that page. That score is an aggregation of critics’ reviews, not a measure of audience approval or an objective verdict on the film.

2. Kimi (2022)
An agoraphobic tech worker hears what appears to be evidence of a violent crime in recorded audio, then runs into resistance when she tries to report it. The threat is both external and psychological: her confinement shapes how she interprets the danger, while the systems around her give her reason to distrust easy assurances. Its contemporary paranoia premise and focused escalation make it one of the tightest entries here.
Rotten Tomatoes’ 2022 annual mystery-and-thriller list displayed a 92% Tomatometer score for the film. The page’s order reflected its own score-based method, not a ranking of psychological thrillers alone.
3. Decision to Leave (2022)
A detective investigating a death becomes emotionally entangled with the person at the center of the case. The film’s romantic-thriller shape is a strength: attraction complicates observation, and each attempt to understand another person creates room for further doubt. It ranks highly for the way emotional obsession and investigative uncertainty work together rather than competing for attention.

Rotten Tomatoes’ 2022 annual list displayed a 94% Tomatometer score. That is a critics’ aggregate reported on that list, not a genre-specific rating.
4. May December (2023)
An actor studies a woman whose past scandal still shapes her family life, and the act of observing and reenacting that life alters the film’s perspective. Rather than relying on a conventional mystery to drive suspense, it builds discomfort through performance, imitation, and the possibility that no one involved has a stable account of themselves.
Rotten Tomatoes ranked it second in its 2023 mystery-and-thriller list and displayed a 91% Tomatometer score. Its critics’ consensus described it as “a seductively discomfiting watch.”
5. The Innocents (2021)
Children discover mysterious abilities during a summer together, and their games take a darker turn. This is a psychological-thriller and horror hybrid with supernatural elements, not a strictly realist story. Its place on the list rests on the way childhood perception, secrecy, and moral choices become the source of unease.
Rotten Tomatoes’ 2022 annual list displayed a 97% Tomatometer score for the film. The score represents critics’ reviews as shown on that page, rather than audience response.

A prison secretary’s quiet routine shifts when she becomes fixated on a new psychologist. Adapted from Ottessa Moshfegh’s 2015 novel, the film works as a close character study: desire and fantasy make an apparently contained life feel increasingly unstable. It is less about a puzzle to solve than about watching obsession reshape someone’s sense of what is possible.

7. The Eternal Daughter (2022)
A daughter and her elderly mother return to a hotel with a connection to their family history. This gothic ghost story uses secrets and family ties to create a slow-burn uncertainty about what the daughter is seeing and what the past means to her. It ranks below the more tightly plotted films for narrative precision, but its lingering ambiguity and atmosphere are its own kind of psychological pressure.
Rotten Tomatoes’ 2022 annual list displayed a 95% Tomatometer score. It is a critic-review aggregate, not a measurement of the film’s psychological-thriller merits specifically.
8. Spencer (2021)
Set during a fraught period in Princess Diana’s life, the film follows a woman under intense public and private pressure. Its psychological-thriller case is interpretive rather than procedural: dread comes from confinement, scrutiny, and a perspective shaped by emotional strain, not from a conventional investigation. That makes it a more interior, less mystery-driven choice than the films above it.

9. Companion (2025)
A gathering at a lakeside house turns unsettling as concealed facts about the relationships come into view. The film combines science fiction with psychological-thriller tension, using uncertainty about what people know and what they are hiding to unsettle the social situation. Its genre-bending premise makes it a useful pick for viewers who want the pressure of a thriller without a strictly realistic setup.
10. Opus (2025)
A young journalist is invited to a retired pop star’s compound for a listening party and begins to suspect that the gathering has a cult-like dimension. The premise puts social manipulation and paranoia at the center, though the available descriptions establish less about its storytelling or critical reception than they do for several higher-ranked films. Its inclusion reflects the fit of that premise with the genre, not a claim that it outranks a fully compared field of fifteen.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to read the ranking
These films cross into courtroom drama, romance, gothic ghost story, horror, and science fiction. The common thread is not a particular setting or plot device but suspense built around uncertain perception, psychological pressure, secrets, or moral doubt. Rotten Tomatoes’ annual lists offer useful context for several titles, but they cover mystery and thriller films broadly and use their own critics’ score methodology; they do not establish a definitive ranking of psychological thrillers.
