The critics we could check gave it a positive reception, and the episode’s strongest case is that the two storylines push each other forward rather than competing for time. Watson’s decline

Watson’s auditory hallucinations and his question about whether someone is causing his decline are the character thread. The synopsis puts these elements in the hour, but it does not establish whether they deepen Watson’s arc or divide attention from the patient story. That judgment belongs to the reviewer, and the critics below read it differently in emphasis, not in outcome.

Moriarty’s return

The official synopsis says only that Moriarty plots his next move. CBR’s Joshua M. Patton goes further, describing this as Moriarty’s first onscreen appearance since the series premiere, with Moriarty threatening Watson’s team while the patient case and Watson’s declining health complicate the hour. That is one critic’s account, not a production fact confirmed elsewhere. Even so, it explains why the return carries weight: it changes the stakes of a case that would otherwise be self-contained.

Does the balance work?

The most useful question about this episode is whether the procedural and the serialized elements reinforce each other. Tell-Tale TV’s Caitlin Wyneken addresses that directly:

“The good news, at least, is that we’re moving in the right direction with the balance of episodic plots and ongoing character arcs.”

Collider’s Maggie Lovitt also reviewed the episode positively, though the listing does not show a numeric score for that review. Taken together, these critics read “Teeth Marks” as an hour where the case of the week and the larger danger are meant to feed one another, with Moriarty’s presence giving the patient story higher stakes and Watson’s decline giving the threat a personal edge.

My verdict: Moriarty’s return is worth the wait if you watched the earlier episodes for the ongoing threat, because this episode puts that threat in direct contact with the team and with Watson’s own unraveling. It is less clear that it will satisfy viewers who want a standalone mystery first, since the synopsis suggests the serialized plot is never far from the surface.
