What Is the “Fourth Hole” on TikTok? What the Slang Means and What the Anatomy Actually Is

“The fourth hole” is internet meme slang, not an anatomical term, and it has no single fixed meaning on TikTok. Its meaning depends on the specific post, its caption, its audio, and the comments around it. The body of the body is anatomy: the human body has three external openings in the area in question, and the sources that describe them do not identify a fourth.

What the phrase means in practice

On TikTok, “the fourth hole” is used as a joke, and the joke usually depends on a deliberate or accidental ambiguity. It is not an official TikTok feature, label, or trend tag, and no platform or medical authority defines it. A 2026 explainer from CloudsPress describes it the same way: a phrase whose meaning shifts with context, not a fixed term that carries the same meaning in every video (CloudsPress, “What is the fourth hole women are talking about TikTok?”).

That means a reader cannot look up one answer and apply it to every video. Some creators use the phrase to tease, some use it to confuse viewers on purpose, and some reuse it because it is already familiar to their audience. The safest reading is to treat the phrase as a punchline whose target has to be established from the post itself.

Where the phrase came from

The best-documented modern meme cycle began on X, not TikTok. Know Your Meme’s guide, dated September 26, 2023, traces the slang to a September 19, 2023 post by the account @t_sadiity: “Cheating on a girl who does all 4 holes is just greedy.” The wording was confusing, and it prompted readers to ask what it meant and to offer competing explanations. Those explanations then circulated across platforms, including TikTok.

The Know Your Meme guide documents that cycle. It does not prove that September 19, 2023 was the first time anyone used the phrase, and it does not establish the first TikTok upload. The 2023 post is the earliest point the guide can document, not a verified origin.

What the anatomy actually shows

MedlinePlus Medical Encyclopedia describes the external openings in the female genital area this way: “The vaginal opening lies just below the urethral opening, and just above the anal opening” (MedlinePlus Medical Encyclopedia, “Normal anatomy”). Those are the openings a medical reference names. The table below lists them in the order they appear from front to back.

One common mix-up is worth correcting. The vagina is an internal canal, and its external entrance is the vaginal opening. The vagina and its opening are one structure described from two angles, not two separate external openings. Counting them separately gives four “holes” in a way that the anatomy does not support.

MedlinePlus does not describe a fourth external opening, and no medical reference cited here assigns the phrase “fourth hole” to any body part. Any claim that the phrase refers to a specific anatomical structure is therefore a claim about the joke, not about the body.

How to decode a specific TikTok

When you see the phrase in a video, the post itself is the only reliable evidence of intent. Check these in order:

  • The caption and on-screen text. A caption that sets up a relationship scenario, a comparison, or a bit usually reveals whether the phrase is a punchline.
  • The audio. Many creators attach the phrase to a sound or a recurring format, and the sound’s original context often explains the joke.
  • The comments. Viewers often ask what the creator means, and the creator’s replies or the most-liked explanations show how the audience reads it.
  • The creator’s other posts. If the creator uses the phrase repeatedly, their pattern is more informative than a single clip.

If none of these settle the meaning, the honest answer is that the video’s intended joke cannot be determined from the phrase alone. Without a specific post, no one can say what a particular creator meant.

What is and is not established

  • Established: the phrase is meme slang; the modern meme cycle is documented back to a September 19, 2023 X post; the phrase’s meaning varies by context.
