Spoilers for the full ending of The Substance. Sue’s rejection onstage triggers the film’s bloody climax, but the sequence is more than a shock ending: it makes public judgment of women’s bodies impossible to ignore. Director Coralie Fargeat describes the final return to Elisabeth’s Hollywood Walk of Fame star as a kind of liberation—an unsettling one that comes only after bodily destruction.
What happens at the end of The Substance?
Elisabeth wakes in pain and disfigured after Sue has repeatedly taken more than her allotted time. She prepares to terminate Sue, then changes her mind. Their breakdown allows both identities to wake at once; they fight, and Sue kills Elisabeth.

Sue returns to the stage for her New Year’s Eve performance, but her body is failing because she has destroyed the balance required by the transformation. She injects herself with the original activating serum and becomes Monstro: a grotesque mass with Elisabeth’s face on the back of her head.
Monstro appears onstage in Sue’s costume. The audience boos and throws objects. The creature’s performance turns into a blood-soaked eruption that overwhelms the stage and the crowd. Monstro tries to escape, falls on Hollywood Boulevard, and breaks apart. Elisabeth’s face survives, crawls to her star, smiles, and dissolves.

Why does Sue become Monstro?
The transformation follows the film’s literal rules: Elisabeth and Sue are supposed to share time in a strict seven-day cycle, and Sue’s repeated extensions accelerate Elisabeth’s deterioration. After Sue kills Elisabeth, injecting the original serum does not restore that balance. Instead, it produces Monstro.
Fargeat’s interpretation connects the transformation to Sue’s desire to be loved as herself, rather than only for her appearance. The crowd has accepted Sue while she fits an ideal; its rejection of Monstro is the point at which the violence Sue has been containing becomes visible. Fargeat described the bloodshed this way: “It explodes in the face of the audience, which represents the world.” That is the director’s stated reading; the film’s extreme spectacle also leaves room for viewers to decide how they respond to it.

What does Elisabeth’s final return to her star mean?
Fargeat describes the last image as relief, happiness, empowerment, and liberation. Elisabeth reaches the symbol of her fame after she no longer has a body to be judged. The ending circles back to the film’s opening use of the star, but it is not a simple happy ending: the release arrives only after grotesque destruction.
The image also completes the film’s concern with control and being watched. Fargeat has said the nudity and exposure were planned with the actors to show vulnerability and the difference between being alone with one’s body and being seen by others. The final public spectacle pushes that contrast to its most violent extreme.

How was the bloody transformation made?
The finished effects combine practical prosthetics and digital work; they should not be described as exclusively practical. France’s Centre national du cinéma et de l’image animée says Fargeat wanted an organic foundation using prosthetics while retaining digital work. Effects designer Pierre Olivier Persin says the script called for Elisabeth’s face on the creature’s back and for its back to split open, while some details developed during effects work. His account also describes practical silicone bodies manipulated from beneath a raised bathroom set for the earlier birth sequence.
The scale of the climax is striking, but its reported figure should be attributed: the Los Angeles Times reported that 30,000 gallons of blood were sprayed from an actual firehose in the sequence.
