Because the story is organized around three generations rather than a single protagonist, readers should expect the narrative to shift between family members and periods. The family history is the spine of the book; the recurring deaths are the question that links the generations.
The recurring drowning
The novel’s driving mystery is a pattern: a member of the family dies by drowning in each generation. In his account, medicine is the vehicle through which the novel explores those themes rather than the subject itself. That framing helps explain why the book is as interested in how people respond to illness, loss and uncertainty as in any single diagnosis.
For readers approaching the novel through its medical strand, the most useful question is not whether a condition can be named, but how characters use clinical knowledge alongside faith and family explanation when the cause remains unclear.

A reading guide: two axes to follow
The publisher’s premise and Verghese’s comments suggest two lines of attention that can organize a reading:
- The multi-generational family mystery. Track how each generation’s drowning is described, what the family knows about earlier deaths, and how the search for an explanation passes from one generation to the next.
- Medical and historical strands. Notice how medical care, belief and the historical setting between 1900 and 1977 shape the characters’ choices and their interpretation of the deaths.
These are interpretive suggestions rather than a settled reading. Faith and love are the themes the publisher and author emphasize, so they are a natural lens for asking how the characters respond to the recurring losses.

The title places art and medicine, and the real and the unreal, in the same phrase.
