What happens at the end of episode 1?
Shuichi wants Kirie to leave town with him. By the closing developments, his concern is no longer just a vague warning: his father’s death links spiral fixation to a real human cost, and Azami’s new forehead mark points to another person becoming involved. The ending turns Shuichi’s fear into evidence, while leaving the cause uncertain.
Why does Shuichi want to leave town?
Shuichi sees a pattern of danger where other people may still see separate oddities. His father’s fate gives that fear weight. The episode offers no confirmed explanation for how the spiral threat works, but Shuichi’s plea to Kirie frames leaving as an attempt to escape what he believes is happening around them.
What does Azami’s spiral mark mean?
The mark gives the episode its forward hook: the spiral is no longer only associated with Shuichi’s father. Its appearance on Azami suggests the threat is reaching another resident. That is a reading of the closing image, not proof of a fully explained supernatural mechanism.

Why is the spiral more than a recurring shape?
The series uses the spiral as both a visible pattern and a sign of escalating obsession and bodily disruption. VIZ describes Junji Ito’s manga as a sequence of connected vignettes in which spirals begin in recognizable forms and become increasingly malicious as they spread through the town. That broader premise supports reading the episode’s ending as the start of an expanding pattern, rather than a self-contained mystery.
The anime’s official synopsis likewise presents spirals appearing across nature, residents and bodies, including a whirlwind, crematorium smoke, spinning eyes, a spiraling tongue and unnatural bodily twists. Those examples establish the wider premise; they do not explain the origin or complete rules of the curse.

Does the ending explain the curse?
No. The official episode summary establishes Shuichi’s warning, his father’s death after becoming fixated on spirals, and Azami’s forehead mark, but does not say what causes the events or how they are connected. The series synopsis describes the spiral’s reach without supplying a definitive mechanism. The ending is therefore open-ended by design in what it establishes: danger appears to be spreading, but its source and limits remain unknown.

Episode 1 is listed by Apple TV as 22 minutes long, released October 4, 2024, with Japan as its region of origin. Viewers who want to compare the anime’s setup and recurring imagery with Ito’s source work can read the manga; it is not necessary to understand the episode’s ending.
