Joel starts out escorting Ellie because the Fireflies believe her immunity may help them find a cure. By the time they reach the hospital, their journey has become a surrogate family bond—and Joel’s decision to save her, then lie about it, leaves that bond resting on a concealed truth.
Why was Joel escorting Ellie?
On outbreak night, Joel’s daughter Sarah is shot and dies while they try to escape. Twenty years later, Joel is a smuggler in Boston’s quarantine zone, guarded by the loss and the dangerous world around him. The death of Sarah shapes how he handles attachment and risk throughout the season.
Firefly leader Marlene arranges for Joel to take 14-year-old Ellie out of Boston. Ellie has been bitten but has not succumbed to infection. The Fireflies see her immunity as a possible lead in the search for a cure, so Joel’s job begins as a delivery, not a rescue mission.
How do the losses on the road change Joel and Ellie?
Tess’s sacrifice
During their escape from infected, Tess is bitten and stays behind so Joel and Ellie can get away. Her death leaves Joel responsible for continuing the journey with Ellie, even though he has not yet accepted her as family.

Bill and Frank, Henry and Sam
Bill and Frank show that connection and a life together are still possible in the devastated world. Later, Joel and Ellie travel with Henry and his younger brother, Sam. Sam is bitten and turns; Henry kills him, then dies by suicide in grief. Their story gives Joel and Ellie another example of how quickly love and loss can become inseparable.
Ellie and Riley
In “Left Behind,” the season returns to Ellie’s past with Riley. They are both bitten, but Ellie survives while Riley does not. The show presents this earlier loss as part of the history Ellie carries into her relationship with Joel. The game The Last of Us Part I also includes the Left Behind prequel chapter, which explores Ellie and Riley’s story.

What happens to Ellie in winter?
After Joel is badly injured, Ellie has to find food and protect them both. She meets David’s group, discovers they are cannibals, escapes, and kills David when he attacks her. Joel finds Ellie afterward. Although they reunite, the ordeal leaves her traumatized and further deepens their reliance on each other.
Why does Joel go to the hospital?
In Jackson, Joel reconnects with his brother Tommy and learns where the Fireflies are. Ellie asks Joel to stay with her, a striking change from the original escort arrangement: the relationship now matters to both of them.

At the Fireflies’ hospital, the doctors believe that examining Ellie’s immunity requires operating on her brain. The procedure would kill her. Joel rescues Ellie, killing the surgeon and others in the hospital, and then kills Marlene.
What happened at the end of The Last of Us season 1?
Joel tells Ellie that the Fireflies had found other immune people and stopped looking for a cure. He knows that is not what happened: the doctors were preparing to operate on Ellie, and he chose to stop them. Ellie asks him to swear his account is true. He does, and her hesitant acceptance leaves the two of them with a truth she has reason to doubt.

The season also gives an explanation for Ellie’s immunity: Marlene says that Ellie’s mother, Anna, was bitten while giving birth and that traces of Cordyceps passed to Ellie. This is the show’s fictional explanation, not a claim about real-world medicine.

Sony’s official series page says Season 2 picks up five years after Season 1. Its broad setup points to Joel and Ellie’s shared past drawing them into conflict with one another and a more dangerous world. The concealed truth about the hospital is the crucial Season 1 history to remember for that handoff.

The television series adapts the story that inspired it; playing the game is optional, not a prerequisite for following the show.
