The episode does not state a cause of death, according to contemporaneous recaps from TVLine and TV Insider. So the direct answer to “How did Cragen die?” is that the premiere does not say.
Why can the offscreen death feel disrespectful?
The criticism is about how the story handles Cragen’s exit, not whether the character mattered. A wake tells viewers he is gone, but the premiere offers no final in-person exchange between him and Benson or the current squad. For viewers who followed Cragen across the franchise, learning of his death after the fact can feel like a notably small farewell.

That reaction appeared in individual fan comments quoted by The Independent and CinemaBlend. One commenter asked why viewers did not get a proper goodbye between Benson and Cragen; another objected to the decision to kill him off. Those posts illustrate a response, but they do not establish how most viewers felt.
What tribute does the premiere give Cragen?
The episode does acknowledge what Cragen meant to Benson. At the wake, she says, “He was the best boss I ever had,” and, “Everything I know about being a captain, I learned from him,” as reported by TVLine and TV Insider. Viewers also see Florek in a video of Cragen speaking at his retirement party, rather than in a new scene as the character.

Those moments make the assessment more complicated than saying the show ignored Cragen’s legacy. Benson’s words explicitly connect his influence to her own leadership, and the video gives him a voice in the farewell. But a verbal tribute and an archival-style appearance are not the same as a final scene with the people he led. Whether the tribute feels sufficient is a matter of interpretation; the absence of an on-screen goodbye is the concrete choice behind the criticism.
Why the “35 years” framing needs context
Cragen’s history reaches back to the original Law & Order, which began in 1990. He later became a series regular on SVU, which began in 1999. Florek stepped down as a full-time cast member in 2014, though he returned to the franchise in later appearances. The 35-year span runs from the original series’ start to the 2025 premiere; it does not mean Florek was a regular on SVU for 35 consecutive years.
