The cave’s antlered tree is the source of both the Sick and the hybrids. When Gus burns it, the immediate outbreak ends, but the finale does not show humanity restored: its epilogue says humans will eventually stop being born and hybrids will inherit the Earth. Old Gus narrates the story to a new generation. Jepperd’s fate, however, remains uncertain.
What caused the Sick?
The finale traces the Sick and the hybrids to the same source: the antlered tree in the cave, and the Blood of the Earth within it. Netflix’s finale explanation says Thacker’s blow against the tree released the Sick against humans and brought hybrids into the world.
Zhang’s actions trigger the tree’s renewed release of the Sick. Gus sets the tree on fire to stop the immediate danger. The humans in the cave recover as the tree dies, but the cave collapses. Singh shields Gus from falling rock and dies.
Does Gus’s act cure the Sick?
It ends the immediate threat, but it does not restore ordinary human births. The finale distinguishes the people who survive the cave from the longer-term future of the species: the humans there recover, while Gus’s later narration says that humans will be the last and hybrids will inherit the Earth. That points to gradual human extinction as hybrids become the only children born—not to every human dying in the cave.

Who is the narrator?
The older narrator is Gus, telling the story to his grandchildren, with an older Wendy beside him. The community-building future he once hoped for has come to pass. His account marks the end of humanity’s era and the beginning of the hybrid generation’s.
The final narration, quoted in Netflix’s Tudum recap, says: “It became clear in the days that followed, that those humans would be the last. Hybrids would indeed inherit the Earth.” Gus then closes with: “All stories end, but ours has just begun.”

What happens to Jepperd?
The finale does not settle whether Jepperd survives. In the present-day frame after the cave, he is wounded and asks whether Big Man made it back. Gus’s story depicts Jepperd returning to the Yellowstone settlement, but that account could be the compassionate ending Gus wants to give him rather than a literal record of events.
Creator Jim Mickle leaves the question to viewers; actor Nonso Anozie also treats it as open. Christian Convery has said he reads the evidence as suggesting Jepperd does not survive. These are interpretations, not confirmation: the final Yellowstone scene deliberately supports uncertainty.

What happens to Zhang?
Zhang’s effort to eliminate hybrids and restore human births helps trigger the tree’s renewed release of the Sick. Gus’s decision to burn the tree stops that immediate danger. The finale does not frame Zhang’s plan as a way to reverse the origins of the Sick and hybrids; instead, it leaves future births to nature.
What does the ending mean?
The finale moves away from the idea that one exceptional person should choose which population survives. Mickle described the outcome this way: “It’s really nature that’s going to decide who goes forward, humans or hybrids.” Gus does not select a species to save; he acts to stop the immediate danger, and the future is left beyond his control.

One reading is that the ending contrasts human attempts to control survival through violence and sacrifice with the possibility of building a community and accepting uncertainty. Gus’s version of Jepperd’s return offers comfort, but the frame preserves the possibility of loss. That is an interpretation of the structure, not proof that Jepperd lived.
Mickle also pointed to empathy as a central emotional aim: “I think we can allow ourselves to feel empathy towards one another and I hope that the show greases the wheels for that a little bit.” The idea fits a finale that gives its characters room for care even as it refuses a fully reassuring answer.

How does the finale fit the last season?
Netflix presented Season 3 as the final chapter. Gus, Jepperd, Wendy and Bear travel to Alaska while Birdie investigates the origins of the Sick; Zhang’s faction seeks to eliminate hybrids and restore human births. Mickle described the season as “an arctic story” and a satisfying conclusion to the tale. The series’ full run is 24 episodes, and Season 3 launched on June 6, 2024, according to Netflix Tudum.
Sweet Tooth is based on Jeff Lemire’s Vertigo comic. The adaptation relationship makes the comic a natural next read for viewers, but the show’s ending should be considered on its own: the available information establishes the source connection, not a scene-by-scene match between the comic and series finales.
