The Ugly Stepsister Review: Gruesome Body Horror With a Point of View

The Ugly Stepsister reframes Cinderella through Elvira, combining dark comedy and graphic body horror to explore the cost of pursuing beauty.

The Ugly Stepsister earns its body-horror warning: its graphic imagery is intense enough that a viewer who recoils—or even feels nauseated—would be understandable. I didn’t throw up, but the discomfort is part of the film’s design. Emilie Blichfeldt’s Cinderella retelling puts the stepsister, Elvira, at the center and turns the pursuit of beauty into a grotesque horror story.

What is The Ugly Stepsister about?

This is not a conventional Cinderella adaptation told from Cinderella’s point of view. Elvira is the protagonist, and the official synopsis centers on her determination to outshine her beautiful stepsister and win the prince’s heart. That shift changes the fairy tale’s emphasis. Rather than treating beauty as a quality that simply grants Cinderella a happy ending, the film follows someone who sees beauty as a condition for being chosen. Elvira’s efforts make the competition for the prince less a romantic contest than a story about what a person may endure to meet an unforgiving ideal.

How graphic is the body horror?

Very graphic. The British Board of Film Classification warns of “sustained and intense horror scenes” and describes a scene involving a large creature being pulled from a woman’s body through her open mouth. The title’s reaction—no vomiting, but an understanding of why someone might—fits the experience better than calling the film merely creepy. Its horror is designed to make the body difficult to look at. If bodily transformation and invasive imagery are hard limits for you, the BBFC description is a useful warning to consider before watching.

What does the horror add to the Cinderella story?

Blichfeldt’s stated “beauty horror” concept draws on body horror and the doctrine “beauty is pain,” according to Berlinale press material. That connection gives the film’s extremity a thematic purpose. Elvira’s pursuit of the prince remains rooted in the fairy-tale premise, but the horror brings the toll of that pursuit into view. The result is not simply a gross-out retelling; it uses disgust to probe the violence implicit in treating beauty as the price of acceptance. The comedy in the film’s mix, as identified by the Norwegian Film Institute, also keeps the story from being only solemn, though the bodily horror remains the most immediate experience.

Who should watch it?

The Ugly Stepsister is best suited to viewers who want a dark, formally bold fairy-tale revision and can handle graphic body horror. Its Elvira-centered perspective and beauty-horror premise offer more than shock value, but the subject matter does not make the imagery gentler. Viewers looking for a mild or family-friendly Cinderella variation should look elsewhere.

The BBFC lists the film at 109 minutes and 21 seconds; Disney+ lists it as 1 hour and 49 minutes.
