Joe Kessler, as he appears beside Billy Butcher in Season 4 of The Boys, is not a living colleague acting alongside him: he is an imagined presence in Butcher’s mind. The show makes that clear in Season 4, Episode 6, “Dirty Business,” when Kessler addresses Butcher’s imagined wife, Becca. Kessler represents Butcher’s destructive impulses; Becca serves as the countervoice urging him away from them.
Is Joe Kessler real in The Boys?
The Kessler in Butcher’s Season 4 scenes is imagined, not an independent person sharing those conversations. That does not mean Butcher never knew a real colleague named Joe Kessler. The twist concerns the figure appearing in the present-day scenes: a real connection to Butcher’s past can coexist with an imagined version of Kessler in his mind.

The reveal comes at the end of “Dirty Business,” when Kessler acknowledges Becca. Because Becca is also a vision, the exchange makes Butcher face that both figures are part of his internal conflict. The scene’s force comes from the two imagined figures speaking to one another, rather than from another character interrupting to ask whom Butcher is talking to.
What does Kessler represent to Butcher?
Showrunner Eric Kripke described Kessler and Becca as “an angel and a devil on Butcher’s shoulder,” dramatizing the human and inhuman sides of Butcher that are at war. Kessler voices the cruel, destructive argument; Becca represents Butcher’s conscience and the case against surrendering to it. Kripke called Kessler “literally the devil on his shoulder,” a description of the character’s dramatic function—not evidence that he is a supernatural being separate from Butcher.

That conflict becomes urgent as Butcher weighs whether to unleash a lethal supe virus. Kessler pushes him toward the catastrophic choice, while Becca argues against it. Kripke said Kessler’s post-reveal role is to twist the knife with cruel truths. His influence is powerful because the voice can exploit things Butcher already fears or knows, not because Kessler is acting in the world as a separate participant.
Why reveal the twist in Episode 6?
Kripke said the writers planned to reveal the truth in Episode 6 rather than save it for the finale. Making the reveal then gave Kessler’s role as Butcher’s inner adversary room to shape the rest of the season. The timing turns the remaining episodes into a more explicit struggle over which part of Butcher will prevail.

The show also supplies clues for a rewatch: Kripke noted that Kessler does not speak to other people and does not really lift things. These are hints, not a complete rule that every imagined figure must follow. The decisive confirmation is the scene in which Kessler recognizes Becca.
How do Butcher’s illness and isolation fit the story?
The visions occur as Butcher faces a terminal illness and grows estranged from the group. The story associates his brain tumor with his use of Compound V, placing the apparitions within the show’s fictional circumstances. This is narrative context, not a real-world medical diagnosis or a general claim about illness, tumors, or drug use.

Does the show leave Kessler’s status unresolved?
No. The main series concluded with its fifth-season finale, released May 20, 2026, according to the Associated Press. Kessler’s role in Season 4 is not an unresolved cliffhanger about a separate living character; the reveal establishes him as part of Butcher’s internal struggle. The series is adapted from the comic by Garth Ennis and Darick Robertson, but this Kessler reveal is specific to the television story.
