Spoilers ahead for Season 1, Episode 4 of The Penguin, “Cent’anni.”
“Cent’anni” shifts the series’ focus to Sofia Falcone, tracing how her suspicions about her father led to ten years in Arkham—and ending with her violent break from the family that helped put her there. The episode makes her past the key to understanding the present, while leaving room to distinguish what happens on screen from what the characters or creators say it means.

What happens in episode four, “Cent’anni”?
“Cent’anni” is the fourth episode of Season 1. Directed by Helen Shaver and written by Lauren LeFranc, it runs 56 minutes according to NOW’s listing. It opens with Sofia injured after the previous episode and calling Dr. Julian Rush; flashbacks then reveal the events that preceded her confinement in Arkham.
Before Arkham, Sofia is publicly associated with a women’s mental-health foundation and presents herself as a philanthropist. A reporter asks about women connected to businesses run by Carmine Falcone who have died by hanging. Sofia recalls finding her own mother’s body in the same manner, and begins to suspect Carmine’s involvement.
#1 Best Overall
Oz, then Sofia’s driver, reports her suspicions to Carmine in an effort to improve his own standing. Carmine turns the investigation back on Sofia. She is arrested for the reporter’s murder and the other deaths, while relatives support the claim that she is unstable. The episode shows her institutional confinement and abuse, including her realization that she will not be released when she expected.
In the present-day story, Sofia returns to the family setting and gives a speech that seems to accept a proposed fresh start. Instead, she uses the gathering to kill most of the Falcone household with gas. Johnny Viti and a child, whom Sofia places safely in the greenhouse, are spared. The episode ends with Sofia taking control through violence; whether revenge, power, or both best explain her choice is interpretation rather than a stated fact.

Why was Sofia Falcone in Arkham?
Sofia suspected her father, Carmine, was connected to the deaths of women who had hanged themselves, including her mother. When Oz told Carmine about those suspicions, Carmine redirected suspicion toward Sofia. She was arrested for the reporter’s murder and the other deaths, and her family backed the account that she was unstable. She then spent ten years in Arkham.
The series presents her confinement not simply as a consequence of the charges, but as an experience of institutional control and abuse. Showrunner Lauren LeFranc told TV Insider that part of the arc’s inspiration was the history of mental institutions and the way women were labeled hysterical or insane. That is LeFranc’s explanation of the creative rationale, not a claim that Sofia’s fictional experience represents every historical case.

Did Oz betray Sofia?
In the flashbacks, Oz tells Carmine about Sofia’s suspicions. The episode establishes that this disclosure contributes to Sofia’s downfall: Carmine turns the investigation against her, and she is sent to Arkham. Oz’s stated motive in the episode is to advance his own position. Calling the act a betrayal is a reasonable reading of his choice, but the concrete plot fact is that he gave Carmine information that helped expose Sofia.
What does Sofia do to the Falcone family?
Sofia uses gas to kill most of the Falcone household during what appears to be a family reconciliation. She spares Johnny Viti and a child, whom she places in the greenhouse. The sequence establishes that she has chosen to seize control by force. It does not reduce her motives to a single, explicit explanation.

How Cristin Milioti and the creative team interpret Sofia’s transformation
Cristin Milioti, who plays Sofia, described the episode’s transformation in a TV Insider interview published October 13, 2024: “What I love about that fourth episode is that you get to see how she was driven mad. She has become the thing that she was accused of being by way of what they did to her.” Milioti’s words describe her interpretation of the character; they are not an objective diagnosis.
Director Helen Shaver recalled reading the script and deciding to direct the episode: “I got to [Episode 4]. I didn’t even have to look at [Episode 5]. I said, ‘I’m doing it. This is a great script. I want to tell this story.’” The official Penguin episode podcast listing identifies a discussion with writer-producer John McCutcheon, editor Meg Reticker, showrunner Lauren LeFranc, Milioti, and Shaver; the listing does not include a transcript.
