Yes—Dr. Cecilia Pederson survives the final gas trap, but the film does not show her escaping. She kills Parker Sears, claims the room’s air opening, and is still trapped when John Kramer, Amanda Young, and Carlos leave.
How does Saw X end?
Set between Saw and Saw II, the film follows John Kramer to Mexico after he is promised an experimental cancer treatment by Dr. Cecilia Pederson and her team. He discovers the operation is a fraud and captures those responsible for his games.
Cecilia’s partner Parker arrives carrying a gun, apparently another patient who has been cheated. The reveal is that he is Cecilia’s lover and accomplice. John has anticipated their connection: he disabled Parker’s gun and prepared a contingency for the viewing area. When Cecilia and Parker go there to retrieve the money, the room seals and fills with poisonous gas, and the blood-waterboarding trap stops.
John tells them only one person can survive by using the room’s air opening. Cecilia kills Parker and puts her head at the opening. She is alive when John, Amanda, and Carlos leave the warehouse, but remains inside the sealed room. The film cuts away without showing a rescue or an escape.

What is confirmed about Cecilia’s fate?
- Confirmed: Cecilia survives the immediate gas test by taking the air opening after killing Parker.
- Confirmed: Parker dies, and John, Amanda, and Carlos leave together.
- Unresolved: The released film does not establish whether Cecilia later gets out or survives beyond the final scene.
Producer Mark Burg told /Film, “We kind of kept her alive in this movie.” That supports the deliberate choice to have her survive the endpoint; it does not answer what happens after the film ends.
Why does John’s final trap spring?
John learns of Parker and Cecilia’s connection after Cecilia calls Parker during the earlier games. While Parker is unconscious, John removes the bullets from his gun. He also sets up the viewing-room contingency. Cecilia and Parker trigger it when they try to recover the money.

The trap reverses the clinic’s exploitation of patients seeking hope: Cecilia, who led the fraudulent operation, is left fighting for a single source of air. John’s plan is calculated, although Carlos’s unexpected arrival is not presented as part of the original plan.
What happens in the mid-credits scene?
The mid-credits scene is separate from Cecilia’s ending. John and Detective Mark Hoffman confront Henry Kessler, the support-group member who first directed John to Cecilia’s supposed treatment. John notices that Henry’s claimed surgery scar is gone, exposing his story as another fraud, and the game begins. The scene does not reveal Cecilia’s later fate.

Where does the ending fit in the timeline?
Saw X takes place between Saw and Saw II, so John and Amanda are alive during its events. Later franchise entries establish their eventual deaths, but those events do not change what the ending of Saw X shows about Cecilia.
In 2025, producer Oren Koules said a then-planned Saw 11 script would begin 30 seconds after Saw X and include Synnøve Macody Lund returning. The same report said the project was cancelled amid disagreements. This was development news, not a scene in the released film, and it does not establish Cecilia’s fictional fate. GamesRadar+ reported Koules’s comments.
