Spoilers for the Shibuya Incident: The figure who looks like Suguru Geto is not Geto acting again. Kenjaku is controlling Geto’s body, and the stitched line across the forehead is the clearest visual clue.
Why does Geto look different?
Shibuya’s apparent Geto has Geto’s familiar face and body, but the person in control is Kenjaku. The difference is a plot reveal, not simply a new design for Geto: the body belongs to Geto, while Kenjaku is the one acting through it.

The official anime site identifies season two as the adaptation of Hidden Inventory / Premature Death and the Shibuya Incident; the season aired in 2023. The official series introduction provides that adaptation context.
What is the clue?
Look at the forehead. The stitched line marks the distinction between the familiar body and the identity controlling it. It is the quickest visual signal that the person facing the other characters is not simply Geto returned unchanged.

Why does Geto’s familiar face matter to Gojo?
Gojo is confronted with the face of someone he knew, even though Kenjaku is behind the encounter. That familiarity gives the reveal emotional force and makes the moment destabilizing as well as dangerous. The face is not incidental: it is part of what makes this confrontation land differently from meeting an unfamiliar enemy.
Is the animation or character design different too?
Viewers may notice differences in linework, shading, styling, or movement between appearances. Those are observations about the anime adaptation, separate from the story explanation. The canon point is that Kenjaku controls Geto’s body; a perceived change in how the scene is drawn does not by itself mean the story has introduced a redesigned Geto.

The relevant identity-reveal stretch is associated with chapter 90. VIZ’s official chapter catalog lists the series and chapter, though reading access may depend on region and availability. Shueisha’s Japanese-language Season 2 official guidebook lists episode commentary, character introductions, and setting materials; its catalog does not establish that it gives a dedicated explanation of this specific reveal.
