Fallout’s ghouls are recurring residents of the wasteland, but the official sources available here do not establish a complete explanation of how they are made or what separates every non-feral ghoul from a feral one. The TV series’ The Ghoul is a specific character—Cooper Howard, played by Walton Goggins—not a stand-in for all ghouls. And in Fallout 76, Bethesda describes a player option to become a ghoul with its own game-specific abilities.
What are ghouls in Fallout?
Ghouls are among the recurring, iconic residents of Fallout’s wasteland. Bethesda’s official Fallout timeline establishes their place in the franchise, but it does not provide a full account of their biology or origins. They appear across the setting; the details of one ghoul’s story should not automatically be treated as rules for them all.
Bethesda says the series takes place more than 200 years after the end of the world as we know it. That is broad setting context, not a measure of how long ghouls live.
Are Fallout ghouls human?

Can you play as a ghoul in Fallout 76?
Yes. Bethesda’s “Beyond the Show: Fallout Season 1” page describes the Fallout 76 update “The Ghoul Within,” which lets players become ghouls. The page highlights radiation resistance and the possibility of a feral melee berserker path. Those are features of this player-facing game update, not proof of universal rules governing every ghoul in Fallout lore. The cited page does not establish exact build comparisons, balance, or patch-specific details.
