The episode gives him a moving confrontation with grief—but survival and a declaration of love are not the same as lasting change. “Down in a Hole” makes a compelling case for Ripley’s capacity to do better while leaving forgiveness, especially Hannah’s, less settled.
What happens in Chicago Med Season 10, Episode 15?
“Down in a Hole” aired March 5, 2025, and is listed as Season 10, Episode 15. Ripley is suspended from Gaffney after his recent conduct. After crashing into an abandoned car, he discovers a mother and daughter trapped in a nearby abandoned well and goes down to help them. They are rescued, but a tunnel collapse leaves Ripley trapped underground. Rotten Tomatoes’ episode guide lists the episode at approximately 42 minutes and TV-14.
Charles keeps talking to Ripley over the radio, challenging his belief that he has given up on himself. Trapped and struggling to survive, Ripley hallucinates Sully, his late friend. Mouch and the firefighters eventually pull him out. The rescue supplies the episode’s physical danger; the conversation with Charles and the imagined encounter with Sully supply its emotional center. Tell-Tale TV’s review reads those moments as a chance for Ripley to recognize how far he has come and begin releasing guilt about Sully’s death.
Does Ripley survive the well collapse?
Yes. Mouch and the firefighters get Ripley out after the tunnel collapse. His survival gives the episode a dramatic resolution to the immediate rescue, but the more consequential question is what he does with the second chance it represents.

Does Ripley finally change?
The episode offers a turning point, not proof of a lasting transformation. Ripley’s willingness to help the trapped family shows decisiveness and concern for others, even while he is suspended. Charles’s words and the hallucination of Sully bring him face to face with grief and self-destructive patterns. Those scenes make his emotional shift legible, but they cannot establish how he will behave after the crisis.
In a March 5, 2025 interview with TV Insider, Luke Mitchell, who plays Ripley, described Sully’s death as the catalyst for Ripley’s recent deterioration. Mitchell said Ripley had been using alcohol to numb and suppress his feelings and falling back into old behavior. He also stressed that Ripley has work ahead to regain Goodwin’s trust. That is the actor’s account of the character’s story, not a clinical assessment—and it supports a reading of this episode as the beginning of accountability rather than its completion.

Does Hannah tell Ripley she loves him?
Yes. Asher reaches Ripley after he is rescued and tells him she loves him. The scene lands as a powerful expression of care, but it does not establish that their relationship is repaired or that Hannah has set aside the harm in their dynamic. The distinction matters: affection can be sincere while trust remains conditional.
That is the episode’s sharpest dramatic tension. A near-death experience can intensify feelings and bring people together; it cannot, by itself, do the sustained work of rebuilding trust. Mitchell called the relationship complicated and said Ripley needs time to work on himself. His post-rescue moment with Hannah is moving, but the episode leaves open whether its emotional acceleration will be matched by change in the days that follow.

Do the side stories strengthen the episode?
Frost’s past intrudes when a former colleague appears and his visibly shaken reaction hints at unresolved history. Asher also receives pregnancy news from her sister, while the episode’s synopsis points to an extremely rare condition discovered through a patient’s ultrasound. These threads widen the episode’s hospital-world texture, though they receive less weight than Ripley’s rescue and emotional reckoning.
The balance is a matter of emphasis: viewers drawn to the central character drama may find the side stories distracting, while those who value the ensemble rhythm may appreciate the competing concerns. The available episode coverage does not establish a definitive medical explanation for the rare condition, so it is best understood here as a subplot rather than a source of medical guidance.
