For the direct print collection match, Shueisha’s Chainsaw Man, Vol. 21 contents include Chapter 198. Its listing gives the Japanese chapter title 「キガちゃんデス!」. No translated title or exact dialogue is needed to explain the reveal, and the available official listings do not establish a verbatim English line to quote.
What does the reveal change about Part 2?
It changes the lens, not the events
Once the presumed Fami is understood as Death, readers can revisit earlier scenes with a different question: were they seeing the motives of Famine, or the actions of a character whose true identity was Death? That shift can make earlier behavior feel more purposeful, but it does not mean every established event is undone or every prior interpretation is automatically false.

That thematic contrast helps explain why the reveal can land as more than a name swap: readers are asked to reconsider a character associated with the story’s plans and conflicts as the figure representing death itself. The exact force of the exchange depends on the licensed chapter’s wording, so it is better not to treat paraphrased dialogue as an official quotation.

Is Chapter 198 really Part 2’s “new peak”?
Calling the chapter a “perfect” twist or Part 2’s “undeniable new peak” is a critical judgment, not an established consensus or measurable fact. The strongest case for that judgment is structural: the reveal revises a core assumption about a major character, gives earlier scenes fresh interpretive tension, and ties the surprise to a larger thematic opposition rather than relying on shock alone.
There is also a reason to leave room for disagreement. A reader may find the chapter’s identity reversal more powerful than its still-open implications, or may want later chapters to clarify how the revelation connects to specific earlier choices. Chapter 198 earns its peak status as an argument about recontextualization; it does not require every reader to rank it above every other Part 2 chapter.
