Hayley McNeff died from acute intoxication involving heroin, fentanyl, 4-ANPP, cocaine and norbuprenorphine, and her death was reported as accidental, according to information attributed to Massachusetts authorities by news outlets in November 2025. McNeff, a bodybuilding champion who appeared in the documentary Raising the Bar, was 37 when she died at her home in Sudbury, Massachusetts, on August 8, 2025.
What was Hayley McNeff’s reported cause of death?
Reports published November 10 and 12, 2025, attributed the findings to the Massachusetts Executive Office of Public Safety and Security: McNeff’s cause of death was acute intoxication due to the combined effects of heroin, fentanyl, 4-ANPP, cocaine and norbuprenorphine. The reported manner of death was accidental. TMZ attributed the information to the state office; People en Español said the office’s Deputy Director of Communications, Brenna Galvin, provided the information to PEOPLE.
These are cause-and-manner findings as relayed by the outlets, not details independently confirmed here from an autopsy or toxicology record. The reports do not publish substance concentrations or explain how the substances were taken.
When and where did she die?
McNeff died on August 8, 2025, at her home in Sudbury, Massachusetts, aged 37, according to coverage citing her obituary. Hindustan Times and Men’s Fitness reported her age and date of death.

Why did early reports say the cause was unknown?
Men’s Fitness reported on August 15, 2025, that no official cause had then been made public. That account preceded the November reports attributing the cause and manner to Massachusetts authorities, so the two reports reflect what was publicly known at different times.
The obituary description of her death as “unexpected but peaceful,” reported by Men’s Fitness and Hindustan Times, was published before the cause was disclosed. It is the family’s wording, not a medical explanation of the death.
