These 15 episodes trace Meredith Grey and Derek Shepherd’s relationship from a one-night stand to a lasting marriage and, much later, a beach reunion. This is an editorial watch list, not an official ranking: the episodes are arranged chronologically by their place in the relationship arc, with the key shared-life milestones followed by a later remembrance. Major spoilers for their relationship, Derek’s death, and Meredith’s COVID-19 illness follow.
First attraction and early complications
1. “A Hard Day’s Night” — Season 1, Episode 1
The story begins with Meredith and Derek waking after a one-night stand. At Seattle Grace, she discovers that the charming stranger is her new colleague—and her superior. That awkward shift from private chemistry to workplace complication sets the terms of their relationship. ABC’s retrospective identifies the morning-after encounter as the moment their story began (ABC’s Meredith and Derek moments retrospective).

2. “The First Cut Is the Deepest” — Season 1, Episode 2
Meredith and Derek have to navigate the hospital hierarchy after their night together. It is an early look at the gap between their flirtation and the professional reality that Derek is an attending and Meredith is an intern. The episode helps establish why their romance is never separate from the hospital where they work.
3. “Who’s Zoomin’ Who?” — Season 1, Episode 9
The relationship takes a sharper turn when Meredith learns Derek is married. The revelation changes the meaning of their earlier intimacy: this is no longer just a difficult office romance, but a choice involving another person and a life Derek has not been honest about. It is essential viewing for understanding the conflict that follows.

4. “Into You Like a Train” — Season 2, Episode 6
Meredith has to live with the consequences of Derek’s marriage and their tangled feelings while facing an intense case at work. Netflix Tudum highlights this episode for the aftermath of Meredith’s plea and the way her judgment as a doctor is tested (Netflix Tudum’s Grey’s Anatomy episode guide). Its medical crisis is central, but the emotional context makes it a useful relationship chapter too.
Separation, choice, and a second chance
5. “It’s the End of the World” — Season 2, Episode 16
A patient arrives with an explosive device lodged in his body, putting the hospital on edge. Meredith and Derek’s strained connection is part of a larger story about trust, fear, and the people who stand beside one another in a crisis. Netflix Tudum includes this episode among the series’ landmarks; watch it as a high-stakes interlude in their turbulent early relationship, rather than as a romance-centered installment.

6. “As We Know It” — Season 2, Episode 17
The bomb crisis continues, and Meredith is placed in extraordinary danger. Derek’s concern for her is unmistakable, but the episode does not resolve the underlying choice between them. Its place on this list is about what their bond looks like under pressure—not a sudden fix for the complications already established.
7. “Didn’t We Almost Have It All?” — Season 3, Episode 25
Meredith faces a defining choice when Derek’s future with her becomes uncertain. Her “Pick me, choose me, love me” plea is one of the couple’s best-known turning points, singled out by ABC among their memorable moments (ABC’s Meredith and Derek moments retrospective). The scene matters because Meredith stops hiding behind indecision and says plainly what she wants.

8. “Freedom, Part 2” — Season 4, Episode 17
Meredith tells Derek she is ready to try again, then uses candles to outline the house they had imagined together. The gesture turns a shared dream into an invitation to build a future. Shondaland highlights the episode in its romance guide, and ABC also recalls the candle house as a signature moment (Shondaland’s Grey’s Anatomy romance guide; ABC’s retrospective).
Commitment and family
9. “Elevator Love Letter” — Season 5, Episode 19
Derek proposes to Meredith in an elevator, surrounded by scans of her medical cases. Rather than making a grand speech in a conventional setting, he frames his promise through the work and life they share. ABC identifies the episode as Season 5, Episode 19 and describes Derek telling Meredith he wants to spend the rest of his life with her (ABC’s guide to classic Grey’s Anatomy elevator scenes). Shondaland also names it as a romantic landmark (Shondaland’s romance guide).

10. “Now or Never” — Season 5, Episode 24
Meredith and Derek’s wedding plans do not unfold as expected. Instead of a city-hall ceremony, they give their wedding to Izzie and Alex and make vows to each other on a Post-It note. ABC describes their Post-It wedding as the unconventional seal on a relationship that began with a one-night stand (ABC’s retrospective); Netflix Tudum also notes Meredith’s choice to make the Post-It their wedding (Netflix Tudum’s episode guide).
11. “With You I’m Born Again” — Season 7, Episode 1
After the hospital shooting, Meredith and Derek confront a new reality: Meredith is pregnant, and the trauma of the previous season has left their future uncertain. This episode brings their partnership into the decisions and vulnerabilities of family life, not just courtship.

12. “What Is It About Men” — Season 8, Episode 4
Meredith and Derek’s home life is shaped by their adoption of Zola, as the episode explores fatherhood and the pressures of raising a child alongside demanding medical careers. It is a worthwhile pick for viewers who want to see the couple as parents, not only as romantic leads.
13. “How to Save a Life” — Season 11, Episode 21
Derek’s death ends their shared life and reshapes Meredith’s story. This is the list’s most devastating entry, and it is best approached as the culmination of the relationship rather than a conventional couple episode. The earlier milestones make its impact—and Meredith’s grief—clearer.

14. “The Center Won’t Hold” — Season 17, Episode 2
While Meredith is comatose with COVID-19, she encounters Derek on a beach in a dreamlike reunion. Their conversation gives the episode its emotional force: he is absent from her waking life, but the connection remains part of how she understands herself. Netflix Tudum identifies this beach meeting as a key moment in the episode (Netflix Tudum’s Grey’s Anatomy episode guide).
15. “You Are the Blood” — Season 17, Episode 14
Meredith’s beach experience continues as familiar faces return, including Derek. The scene belongs to a later remembrance arc, not to the couple’s life together, but it offers another moment of connection after his death. It works best after “The Center Won’t Hold,” when the beach’s role in Meredith’s recovery is already clear.
