What happens to Elisabeth and Sue?
Elisabeth and Sue are two forms of the same person, required to alternate every seven days. Sue’s success leads her to take extra time, using more stabilizing fluid and accelerating Elisabeth’s deterioration. Elisabeth decides to end the arrangement, but hesitates; both forms wake at once, and their confrontation ends with Sue killing Elisabeth.

With Elisabeth dead, Sue attempts to use the original activation serum to recover her earlier appearance before the televised New Year’s Eve show. The attempt does not restore her. It transforms her into a grotesque, unstable figure that combines elements of both women. Sue goes onstage anyway, but the audience recoils. She escapes the studio and breaks apart on Hollywood Boulevard. Elisabeth’s face survives briefly, reaches her star on the Walk of Fame, and dissolves.
What does the final image mean?
Fargeat has described Elisabeth’s return to her star as a moment of liberation: without a body, Elisabeth no longer has to worry about how that body looks. She has also called the ending hopeful. That hope belongs to the director’s symbolic reading of the last image, not to the characters’ physical fate: neither Elisabeth nor Sue survives.

The crowd’s reaction makes the film’s judgment of appearances public and unmistakable. Sue has been celebrated for her youthful image; once she becomes visibly monstrous, the same audience rejects her. Fargeat has said the crowd represents society, so its response reflects the social judgment that has shaped both women’s self-worth. The last image can be read as release from that judgment, rather than a reunion or a literal return to life.
Why does Sue’s transformation fail?
The serum’s rules are part of the film’s invented body-horror premise, not real medical science. The two forms depend on an arrangement of alternation and stabilization; Sue’s overuse has already damaged Elisabeth and destabilized their relationship. After killing Elisabeth, Sue tries to reverse the consequences by repeating the initial activation, but the result is not a reset. The transformation produces an unstable hybrid rather than the youthful appearance she wants.

Fargeat has said she wanted the monster to evoke tenderness as well as horror: “She wants to be loved for who she is, not what she looks like.” Sue’s insistence on performing despite her transformation makes that desire visible, while the audience’s rejection shows how conditional its earlier approval was.
What themes does the ending emphasize?
Fargeat has described the film as “fundamentally about the violence of control.” In an interview with Vogue, she connects that idea to expectations that women remain delicate and controlled, and frames the extreme ending as a way of shattering those constraints. Working Title Films quotes her describing the film as a response to the long-standing pressure to control and hold back women’s bodies.

On that basis, the ending supports a reading of The Substance as a critique of a culture that rewards Sue’s youth and beauty, then turns away when she no longer fits the image. Elisabeth’s final release is not a happy escape from the events of the story; it is the film’s surreal imagining of freedom from a body and an appearance that have been relentlessly judged. That reading is grounded in Fargeat’s explanation, but it need not be the only way a viewer interprets the final image.
