In the 2024 prequel, Margaret is Damien’s unwilling human mother. A radical faction within the Church engineers his birth, then places the boy with Robert and Katherine Thorn after their newborn dies. Margaret escapes with Carlita and Damien’s surviving twin sister—a reveal that adds another child to the story but does not confirm a sequel.
How the ending unfolds
Margaret discovers the conspiracy includes her
Margaret’s investigation into the orphanage leads to the truth about her own past: she was born through the jackal-linked breeding scheme and selected to be impregnated by the same demonic figure. The plot is not simply an attempt to create an Antichrist in the abstract; it is a deliberate violation of Margaret, whose body the conspirators intend to use.
A faction within the Church wants to create the Antichrist
Father Brennan explains the faction’s stated reasoning. Its members fear secularism and believe the Antichrist’s arrival will frighten people back toward religion. This is the motive of the fictional extremists in the film, not an assertion about the historical Catholic Church.
Margaret gives birth to twins
The conspirators expect a male Antichrist, but Margaret gives birth to a girl as well as a boy. They identify the boy as Damien and prioritize him. The girl’s birth is the climax’s surprise: she survives rather than disappearing from the story.

Damien is sent to the Thorns
Damien is placed with Robert and Katherine Thorn to replace their dead newborn. That handoff supplies the origin story for the child at the center of the 1976 film. Margaret and Carlita escape with Damien’s twin sister.
Why would the conspirators create the Antichrist?
The faction believes that fear can restore religious belief: if people see the Antichrist’s arrival as proof of evil, they may turn back to religion. That is the characters’ explanation for the scheme, rather than a claim that the Church as a whole supports it. The film thus makes the conspiracy an extremist project operating within the institution, and puts Margaret’s experience at the center of its cost.

What the twin sister changes
The ending leaves two surviving children on separate paths. Damien is headed for the Thorn family; his sister survives with Margaret and Carlita. The film does not explain what becomes of the girl after their escape, so her future remains open within the story.
Actor Nell Tiger Free described the second child as a female counterpoint to Damien and said the reveal opened up possibilities. Director Arkasha Stevenson has also discussed Sister Silva and the jackal’s backstory as avenues she found interesting. Those comments describe possible directions, not an announced continuation: the film itself does not confirm a sequel.

How the prequel changes the 1976 film’s mythology
The First Omen is presented by 20th Century Studios as a prequel to the 1976 film, but its account of Damien’s conception differs from the earlier story. Den of Geek’s ending analysis describes the original film’s account as Damien being born from a female jackal; the 2024 film instead makes Margaret his unwilling human mother after an assault by the jackal figure. It also gives the conspirators a different emphasis: the prequel identifies them as a radical Church faction seeking to revive belief through fear of the Antichrist.
| Story element | The Omen (1976) | The First Omen (2024) |
|---|---|---|
| Damien’s birth | The original account describes him as born from a female jackal. | Margaret is impregnated by the jackal figure and gives birth to Damien and his twin sister. |
| Conspirators | The prequel’s account of the original frames the group as Satanist infiltration. | A radical faction within the Church seeks to revive religious belief through fear. |
| Story’s point of view | The story centers on the Thorn family’s discovery. | The story centers on Margaret’s experience of the conspiracy. |
These are changes in the prequel’s interpretation of earlier lore; they should not be treated as proof that every installment in the franchise follows one perfectly consistent continuity. Stevenson said the film was intended as a prequel while also working as a story in its own right. Producer David S. Goyer described the question driving the origin story as how Damien came to be placed with Robert and Katherine Thorn.

Why the ending works as an origin story
The film answers the practical question behind Damien’s appearance in the Thorn household: the couple’s baby dies, and the conspirators use Damien to replace him. But it shifts the emotional perspective from the family discovering what their child is to Margaret discovering what has been done to her. Stevenson has said she did not want the film to be only a creepy-child story; she described it as being more inspired by Repulsion and focused on one woman’s journey.
That focus makes the twin reveal more than a detail about Damien’s birth. The boy’s placement completes the link to the original film, while the girl’s survival leaves Margaret’s story open beyond the conspiracy’s immediate plan.
