Nurse Ratched is the head nurse of the psychiatric ward in Ken Kesey’s 1962 novel One Flew Over the Cuckoo’s Nest, where patients call her “Big Nurse.” She becomes the central institutional adversary of Randle McMurphy, a new patient who challenges her control of ward life. Louise Fletcher played Ratched in Miloš Forman’s 1975 film adaptation.
Who is Nurse Ratched?
Ratched oversees the ward’s routines and the patients’ group meetings. Her authority shapes what happens there and what patients are expected to disclose. McMurphy’s disruptive behavior puts that authority under pressure, making their conflict a struggle over control, conformity and personal autonomy.
The novel is narrated by Chief Bromden, a patient, so readers encounter Ratched through his perspective rather than through an impartial biography. The story establishes her role in the ward; it does not offer a complete account of her life before it.
Is Nurse Ratched’s first name Mildred?
“Mildred” is not a first name established for Ratched in Kesey’s novel. Louise Fletcher said she made up the name for the film, and Jack Nicholson used it in a scene. The book calls her “Big Nurse,” the patients’ name for her.

Who played her in the film?
Louise Fletcher portrayed Ratched in Miloš Forman’s 1975 adaptation. The film helped make the character widely recognizable, and Fletcher’s performance cemented her reputation as a screen villain. Fletcher said she did not want to play Ratched as a monster: “I didn’t want to make her a monster – I wanted to make her believable as a real person in those circumstances.”
Fletcher also described seeing how people can believe they are doing good and know best. That perspective connects the performance to a broader concern with people who misuse power while considering their actions justified.

How does the novel differ from the film in portraying Ratched?
| Aspect | Kesey’s 1962 novel | Forman’s 1975 film |
|---|---|---|
| Point of view | Narrated by Chief Bromden, a patient; Ratched is seen through his perspective. | Fletcher portrays Ratched on screen; the film brought the character to a wider audience. |
| Name | The patients call her “Big Nurse”; the novel does not establish “Mildred” as her first name. | Fletcher said she invented “Mildred,” which Nicholson used in a scene. |
| Backstory | Her life before the ward is largely obscure. | Fletcher said she knew little of Ratched’s history and aimed for a believable portrayal. |
These differences support a comparison of perspective and naming, but do not by themselves amount to a scene-by-scene account of the adaptation.
What does Nurse Ratched represent?
A common interpretation treats Ratched as a figure of institutional order and coercive authority, set against McMurphy’s resistance. That reading highlights the novel’s conflict between individual autonomy and an institution’s power to impose routines and expectations.

Her portrayal is also debated. Feminist critics, including Philip Darbyshire in a 1995 article, have challenged the way Kesey depicts a powerful woman whose authority and sexual guardedness are central to the conflict. This is a critical interpretation, not a single meaning that all readers share. The story can be read as a clash over institutional power while also raising questions about the gendered terms in which that clash is presented.

The sources discussed here do not establish a full biography of a real nurse behind the character. TIME relayed Ken Kesey’s retrospective recollection of encountering a nurse who helped inspire Ratched: “She was much smaller than I remembered, and a whole lot more human.” It is Kesey’s reported remark, not independent verification of that nurse’s identity or life.
