Liam Payne Cause of Death: Polytrauma and Internal Bleeding After Hotel Balcony Fall

Liam Payne died from polytrauma—multiple severe traumatic injuries—with internal and external hemorrhage after falling from the third-floor balcony of the CasaSur Palermo Hotel in Buenos Aires. The fall, rather than a drug overdose, was the mechanism that caused the fatal injuries. Later toxicology found alcohol, cocaine-related substances, cocaethylene and sertraline in his system, but those findings were treated as possible contributing circumstances—not as the stated medical cause of death.

Payne died on October 16, 2024, in the internal courtyard below room 310. The medical cause is established in the Argentine forensic findings and was also recorded at the opening of the UK inquest. However, as of August 10, 2026, the UK coroner’s final conclusion about exactly how he came by his death remains pending.

The short answer: what killed Liam Payne?

According to Argentine forensic findings, Liam Payne’s cause of death was polytrauma and internal and external hemorrhage caused by the injuries he sustained in a fall from a hotel balcony. A preliminary autopsy reported 25 injuries and said the head injuries alone were sufficient to cause death. The later toxicology findings added important information about his condition before the fall, but did not replace the medical conclusion that the fatal injuries were traumatic.

That distinction matters because several different questions have been compressed into the phrase “cause of death”:

What does “polytrauma” mean?

Polytrauma is a medical description, not a separate disease or a euphemism for poisoning. It means that a person has sustained multiple serious traumatic injuries, potentially involving several parts of the body and internal organs.

In Payne’s case, the term refers to the extensive injuries caused by the fall. The associated internal and external hemorrhage describes the severe bleeding that followed those injuries. The preliminary autopsy also stated that the head injuries were independently serious enough to cause death. The UK inquest opening later recorded polytrauma as the medical cause of death as well.

Because the full underlying autopsy and toxicology reports have not been made public in their entirety, detailed information in the public record comes largely from prosecutor filings, court summaries and reporting on the preliminary autopsy.

What happened at the CasaSur Palermo Hotel?

Payne was staying at the CasaSur Palermo Hotel in Buenos Aires during the period later examined by investigators. On October 16, 2024, he fell from the balcony of room 310, on the hotel’s third floor, into an internal courtyard below. An emergency call was made at approximately 5:10 p.m. local time, and emergency medical personnel confirmed his death at the scene.

The official materials establish that Payne died after a fall. They do not establish, as a settled fact, that he deliberately “jumped.” Some early emergency-call wording and headlines used language that could be read that way, but the forensic and court records investigated the circumstances of the fall. For that reason, “died after falling” is the more accurate wording unless a different description is directly attributed to an early source.

What did the autopsy find?

The preliminary autopsy reported 25 injuries, all attributed to the fall, along with internal and external bleeding. It described the injuries as compatible with a fall from height and said the head injuries were sufficient to cause death. The findings were reported on October 17, 2024, one day after Payne died.

Later Argentine forensic material said there was no evidence of self-inflicted injuries and no evidence that another person had physically intervened in a way that caused the injuries. Those findings address important questions about the physical evidence, but they do not by themselves answer every question about Payne’s intentions or state of mind.

Graphic descriptions and leaked images of Payne’s body should not be treated as legitimate evidence or circulated. A Buenos Aires court issued a preventive order prohibiting the publication or dissemination of images and videos showing his body or his physical condition shortly before his death. The court’s explanation is available through the Buenos Aires City judiciary.

Did drugs cause Liam Payne’s death?

The public forensic record does not describe Payne’s death as an overdose. The stated medical cause was the polytrauma and hemorrhage resulting from the fall.

That does not mean the toxicology findings were irrelevant. A later Argentine prosecution filing reported that Payne had alcohol, cocaine and cocaine-related compounds, cocaethylene and sertraline in his system. Forensic doctors said the findings could have produced a state of mental alteration and could help explain his condition before the fall.

The careful conclusion is therefore:

  • The fall caused the fatal traumatic injuries and hemorrhage.
  • Substances may have contributed to Payne’s impaired condition.
  • The public evidence does not prove that a particular drug directly caused the fall.
  • The presence of drugs is not the same as a drug overdose.

Any statement that intoxication caused the fall should be presented as an investigative or prosecutorial theory, not as the settled medical cause. Toxicology identifies substances in the body; it does not automatically establish exactly how those substances affected a person’s decisions, balance or consciousness at a particular moment.

What exactly did toxicology find?

The June 2025 Argentine filing described the following findings:

  • Ethanol: up to 2.7 grams per liter of blood.
  • Cocaine.
  • Metilecgonine and benzoylecgonine, which are cocaine-related compounds reported in the toxicology results.
  • Cocaethylene.
  • Sertraline, a prescription antidepressant.

The forensic doctors said the cocaine and its metabolites indicated use beginning at least 72 hours before Payne’s death and potentially continuing until shortly before his death. They also reported nasal and inhaled routes of cocaine use. The details come from the Argentine prosecutor’s filing seeking an oral trial for two alleged suppliers.

These results should not be relabeled as an official finding of “pink cocaine.” That phrase is a media and street-market label whose contents can vary; it is not the wording used in the cited official forensic filing. The public filing identifies cocaine, its metabolites, cocaethylene, alcohol and sertraline. It does not establish “pink cocaine” as a toxicological category or confirmed cause of death.

Was Liam Payne conscious when he fell?

Argentine prosecutors and forensic doctors said Payne did not appear to show a normal defensive or protective reflex during the fall. Prosecutors inferred from that evidence, together with the toxicology findings, that he may have been semi-conscious or unconscious, or otherwise in a state of substantially diminished consciousness.

That wording needs to remain qualified. The absence of a normal protective posture may support an inference about a person’s condition, but it does not prove the precise level of consciousness, what Payne was thinking or whether he intended to fall. The forensic evidence is strongest on the physical cause of death; the mental and situational circumstances remain a matter for the investigations and inquest.

Was his death suicide, murder or an accident?

No single label should be presented as the final official answer across all jurisdictions and stages of the proceedings.

The Argentine forensic reports found no evidence of self-inflicted injuries or physical intervention by another person. Prosecutors said the evidence indicated Payne was not fully conscious and therefore did not make a conscious or voluntary act. Those findings point away from a straightforward account of deliberate self-harm or a physical attack, but they are not the same as a completed UK coroner’s determination that the death was legally “accidental.”

The distinction is important:

  • “Suicide ruled out” is too broad if it suggests that every question about intention has been conclusively resolved. More accurately, the Argentine forensic findings did not support self-inflicted injuries, and prosecutors said the evidence pointed away from a conscious voluntary act.
  • “Murder ruled out” is also too broad as a summary of every legal issue. More accurately, the forensic doctors reported no evidence of physical intervention by a third party.
  • “Accidental death” should be attributed to a specific authority or avoided as a definitive label while the UK inquest remains incomplete.

The physical cause—trauma and hemorrhage from the fall—is much clearer than the final legal or coronial classification of how the fall occurred.

What is the status of the UK inquest?

The UK inquest opening, reported in December 2024 and January 2025, recorded polytrauma as Payne’s medical cause of death. The proceedings were then adjourned while the coroner sought evidence from Argentina, including forensic reports, eyewitness statements and other material needed to examine the circumstances and manner of death.

At a pre-inquest review on May 7, 2026, the coroner said he had received no further disclosure from Argentina through the UK Foreign, Commonwealth & Development Office and was still awaiting evidence about how Payne came by his death. A further review was reported as scheduled for December 1, 2026.

As of August 10, 2026, the final UK inquest had therefore not concluded. The ITV News report on the May 2026 review explains the outstanding evidence. The medical cause recorded at the opening is not in conflict with the Argentine findings; the unresolved issue is the broader question of how and in what circumstances the fatal fall occurred.

What happened in the Argentine criminal case?

The criminal proceedings concern allegations about drug supply and, at earlier stages, alleged negligence or abandonment. They should not be confused with the forensic determination of what physically killed Payne, and allegations are not convictions.

November 2024: three people charged

On November 7, 2024, Argentine prosecutors publicly described the toxicology findings and announced charges against three people. The investigation included allegations relating to abandonment of a person followed by death, as well as the supply or facilitation of narcotics.

February 2025: proceedings narrowed

On February 19, 2025, an Argentine appeals court dismissed negligent-homicide proceedings against businessman Rogelio “Roger” Nores and two hotel employees, while upholding proceedings against two alleged drug suppliers. The court’s summary is available from the prosecutor’s office.

June 2025: prosecutors sought an oral trial

Prosecutors sought an oral trial for Braian Nahuel Paiz and Ezequiel David Pereyra, alleging paid supplies of cocaine on October 14, 15 and 16, 2024. The filing described the alleged drug-supply conduct; it did not establish that either man medically caused Payne’s death.

March 2026: the two alleged suppliers were released

On March 25, 2026, a Buenos Aires City judge ordered Paiz and Pereyra released. The judge noted that the investigation was considered concluded and that prosecutors had not identified a causal link between the men’s conduct and Payne’s death for the purpose of continued detention. Release did not itself constitute an acquittal or conviction. The judiciary’s account is available here.

June 2026: Nores’s dismissal confirmed

On June 30, 2026, Argentina’s Criminal Cassation Court confirmed Nores’s dismissal from the drug-supply allegation, according to reporting on the ruling published by Infobae.

These procedural outcomes change who remains legally involved in particular allegations. They do not change the forensic cause of death: the fatal injuries and hemorrhage followed the fall.

Why the early reports and later findings are not contradictory

Early coverage focused on the preliminary autopsy: multiple injuries, head trauma and internal and external bleeding. Later coverage added the results of toxicology and other forensic work. Those are different parts of the investigation.

The preliminary autopsy answered the immediate medical question of what injuries killed Payne. The later filing provided additional information about substances detected in his body and their possible relevance to his condition before the fall. Toxicology did not transform the cause of death into an overdose, nor did it eliminate the importance of the traumatic injuries.

Similarly, the phrase “cause of death: fall” is understandable in ordinary language but medically incomplete. The fall was the mechanism; the polytrauma and hemorrhage were the medical cause. Whether the fall was accidental, voluntary or otherwise brought about is a separate question about manner and circumstances.

What can and cannot be concluded?

Supported by the public record:
