The medical case

Sherlock’s return
The premiere also picks up ongoing character threads, and it closes by setting up Sherlock’s return. The Guardian’s February 15, 2026 review notes his appearance at the end of the episode. Is the “House” comparison fair?
Calling Watson a “House replacement” is an editorial interpretation, not a claim made by CBS. The show is a medical procedural centered on Dr. John Watson and a team that investigates rare diseases, so the comparison to House’s diagnostic-mystery format is reasonable on premise. Reviewers have made the same link: The Guardian calls the series a cross between House and Elementary, and the A.V. Club compared its setup with House.
The comparison breaks down on focus. Watson carries Sherlock Holmes characters and mythology that House never had to manage, and that is the tension most reviewers of this premiere return to.

Does the “predictable” label hold up?
The headline’s word “predictable” comes from the A.V. Club’s January 2025 review of the series pilot, where Samantha Nelson criticized the dialogue and story beats as predictable. That is a critic’s opinion about the first episode of the show, not a finding about the Season 2 premiere. Wilson’s recap, which covers this episode directly, objects to clunky writing and a divided focus, but the sources cited here do not call the Elizabeth storyline predictable. A reader who wants to test the claim should judge the donor reveal and the Holmes ending against their own expectations.
