Episode 3 of Fallout Season 1, “The Head,” is a delivery story that keeps failing. Lucy is trying to carry Wilzig’s severed head across the Wasteland, and nearly every attempt is interrupted by something more immediate: a swallowing creature, a captor who needs medication, a damaged suit of power armor. What happens in “The Head”
The episode runs three threads side by side: Lucy’s attempt to deliver the head, Maximus’s attempt to keep his power armor working while he pretends to be the Brotherhood knight Titus, and a pre-war flashback to Cooper Howard’s acting career and his link to Vault-Tec. Prime Video lists the episode at 57 minutes, with a release date of April 10, 2024, and an 18+ rating in the listing consulted. Ratings and catalog details can differ by territory.
Lucy and the tracked head
Lucy keeps moving with Wilzig’s head, which she has fitted with a tracker. A gulper swallows it. The Ghoul picks up Lucy’s trail, takes her captive, and plans to use her as bait to recover the head. Lucy fights back, but the struggle is repeatedly cut short by the Ghoul’s depleted medication, which becomes a practical problem for both of them rather than a plot device that can be waved away.
PC Gamer’s recap quotes the episode’s golden-rule line as “Thou shall get sidetracked by bullshit every goddamn time.” The official Prime Video synopsis uses a cleaner tagline instead: “The Wasteland’s got its own Golden Rule…” The longer line is the recap’s attribution, not promotional copy.

Maximus, Titus, and the armor
Maximus needs a part to repair his power armor and sells a tooth to pay for it. Scavengers try to take the armor, and he has to fight them off. His earlier deception, posing as Titus, leads the Brotherhood to send a replacement squire. That squire is Thaddeus, a former bully, and the two are now uneasily partnered.
Maximus and Thaddeus track down the gulper. Maximus saves Thaddeus during the fight, and Wilzig’s head is recovered from the creature. The pair also take in CX404, the Ghoul’s dog, according to the detailed secondary recap used for this summary.

Cooper Howard before the war
The flashback follows Cooper Howard as an actor. PC Gamer describes a film-set conversation about giving his character a darker turn, followed by a Vault-Tec promotional shoot in which Cooper gives the familiar thumbs-up. The episode ties that image to the Vault Boy mascot, which helps explain the Ghoul’s hostility toward the symbol. The episode presents this as a character and franchise connection; it does not, on its own, settle the wider history of Vault-Tec.
